CARE COORDINATOR

POSITION DESCRIPTION

General Statement of Duties: The Care Coordinator (CC) is responsible for the provision of quality care management for Puget Sound Kidney Centers (PSKC) patients and documentation following established PSKC standards and practices and in compliance with the Washington State Nurse Practice Act. This position description is not an employment contract. Management has the exclusive right to alter this position description at any time without notice.

QUALIFICATIONS

Education: Evidence of graduation from an accredited school of nursing. Bachelor’s Degree in nursing preferred.

Licensure & Certification: Current licensure from the Washington State Board of Nursing.

Experience:

• Minimum of two years’ experience in Acute and/or Chronic Hemodialysis.

• Prior experience with patient education preferred and case management preferred.

Skills & Abilities

( Able to differentiate color schemes.

• Read, write, speak, and understand English for the purpose of following written policies and procedures, documentation, and communicating with others in a work setting.

• Ability to work flexible eight (8) hour schedules, five (5) days per week (Monday through Friday). Occasional Saturday shifts may be required. Able to work flexible shifts between 7:00 a.m. and 7:00 p.m.

• Ability to maintain excellent attendance and punctuality.

• Ability to work at any PSKC facility and complete work remotely as needed.

• Has successfully completed initial dialysis staff nurse training and preceptorship.

• Ability to utilize every type of dialysis access with ease with an understanding of their functions, and teach others to analyze and use dialysis accesses appropriately.

• Ability to understand and/or implement all modalities of dialysis safely and competently.

• Can maintain up-to-date records in a neat and orderly manner.

• Approach work requirements in a consistent, methodical, well-organized, and timely manner.

• Can react calmly and effectively in emergency situations.

• Can maintain quality control and infection control standards.

REQUIREMENTS

Physical Demands

▪ Regularly required to stand, sit, and walk during work hours.

▪ Lifting, positioning, or physically moving patients.

▪ Pushing wheelchairs and dialysis/medical equipment.

▪ Frequently reaching, bending, kneeling, or stooping, and other range of motion activities.

▪ Lifting 30 lb. containers or boxes.

▪ Viewing a computer screen for two or more hours at a time.

▪ Visual acuity to discern changes in patient’s skin and/or blood color, differentiate between colors of medication and/or solutions, interpret typed or handwritten documentation, and identify labels appropriately.

▪ Hearing acuity, within correctable to normal range, to communicate effectively with others, to discern changes in patient conditions and to hear equipment alarms.

▪ Manual dexterity sufficient to manipulate medical records, equipment, and perform other essential job functions.

▪ Periodically required to work long hours and occasional weekends.

Other Requirements

( Knowledge in the operation of all equipment and supplies used in caring for the dialysis patient.

• Maintain confidentiality of records, patient, employee, hospital or PSKC information at all times.

• Ensure all information and conversations regarding patients are kept confidential from the public.

• Adhere to policies and procedures of PSKC for delivery of quality nursing care.

• Support the patient’s right to safety, privacy, and confidentiality.

• Maintain current knowledge of patients and the care provided by the Puget Sound Kidney Centers.

• Adhere to PSKC’s general administrative, human resources, and departmental policies and procedures and upholds the standard of professional nursing practice.

• Continue to improve professional competence through related literature, attendance at PSKC in-services and external conferences as approved, and maintain a personal education profile.

( Responsible to the PSKC patient and the patient's physician for delivery of quality nursing care, as defined by PSKC Officers, Directors, and Department Heads.

EQUIPMENT USED

• Dialysis machines and various equipment and supplies related to the dialysis process.

• Computers and computer operated equipment.

• Standard office equipment and supplies (phone, fax & copy machines, stapler, file folders, etc.)

WORK ENVIRONMENT

( Regularly exposed to the risk of bloodborne, respiratory and airborne diseases.

( Exposure to industrial and pharmacological hazards as defined in various Material Safety Data Sheets (MSDS).

( Contact with patients under a wide variety of circumstances; including unpredictable situations.

( Dialysis clinic is typically quiet with occasional period of heightened traffic through the work area, increased noise level, and interruptions.

ESSENTIAL FUNCTIONS

( Ensure that the dialysis needs of the patients are met.

• Provide case management, for approximately 100 patients that include, but are not limited to: hospitalization/re-admission tracking and prevention, care transitions management, access management, lab evaluation, medication titration and reconciliation, MD communication, infection tracking/prevention and specialized patient and staff education.

( Initiate hemodialysis treatment, according to PSKC procedure, when needed for access management or educational opportunities.

( Maintain daily clinical records with accurate, complete and legible documentation.

( Provide initial and follow-up patient education in all aspects of hemodialysis and related topics.

( Develop and maintain accurate, complete and timely patient assessments and care plans as required.

( Provide appropriate intervention during an emergency situation.

( Provide leadership and education to Dialysis Technicians and Staff Nurses.

( Maintain accurate, complete and current orders on assigned patients.

( Attend patient care conferences when scheduled as part of the interdisciplinary team (IDT).

( Maintain open communication between patient and IDT. Refer patient needs to IDT as necessary.

( Participate in orientation and training of new employees as needed.

OTHER FUNCTIONS

Interpersonal Relationships: Frequent interaction with co-workers, patients, physicians and their medical office staff, and occasional interaction with family members.

NOTICE REQUIREMENTS: To resign this position in good standing, the Care Coordinator must give thirty (30) days notice, in writing, to the Director of Nursing or Direct Care Manager. Employees who have successfully completed their orientation period and provided this advance notice of resignation will receive the value of any accrued Personal Time-Off (PTO).
