This position is responsible for providing clinical expertise to nursing and health care providers in the public and private sectors to improve the clinical health outcomes of people living with HIV. This position is also responsible for serving as an expert resource to the Division of HIV and STD Programs in the development of policies and standards of clinical HIV care.Duties and Responsibilities:Serve as the HIV Clinical Care Coordinator to provide guidance to funded agencies in the development of innovative HIV health care coordination programs.In collaboration with the division’s evaluation and quality management staff, develop program and evaluation criteria, work plans, quality assurance standards, policies and protocols to support sub-recipients.Provide training and guidance to sub-recipients on the rules, policies and procedures related to the federal Ryan White legislation.In collaboration with MDHHS HIV Surveillance and CAREWare Data Team, assist sub-recipients in the development of baseline reports, data reporting and monitoring of trends throughout the project period.Oversee the development, implementation and evaluation of a Request for Proposal process for care coordination programs.Assist with grant applications and progress reports; maintain records, reports and correspondence related to work.Facilitate conference calls with sub-recipients to enhance collaboration and share lessons learned.Perform site visits to monitor performance and create site visit reports of findings and recommendations. Provide health care instruction and education to local agencies related to clinical HIV care and improvement of health outcomes.Provide clinical consultation and serve as an expert resource to the Division of HIV and STD Programs in the development of policies and standards of clinical HIV care; adapting the procedures and practices to meet the changing needs of health care programs.Assess trends and developments in clinical and public health care and nursing; identify program needs, and project the impact of changes on programs and services.Participate in and collaborate with the Michigan HIV Nurse Network and project the impact of changes on programs and services. Liaise with other professionals, community agencies and HIV planning councils relative to program review, appraisals and problem resolution.Participate in all required meeting and trainings, including confidentiality, security trainings and oath of confidentiality.Other duties as assigned by administration.Qualifications/Requirements:Education: Possession of a bachelor’s or master’s degree in nursing accepted for licensure by the Michigan Board of Nursing.Experience: Required: Two years of professional experience in a position responsible for the implementation or direction of health programs. Highly Desirable: Experience working with vulnerable or underserved populations, community organizations and public health.Important Skills and Characteristics: Commitment to health equity and serving the underserved populations in Michigan. Standard office environment. Requires valid vehicle operator’slicense where needed to perform duties of the position. Frequent deadlines.
