CAPITOL REGION EDUCATION COUNCILPOSITION DESCRIPTIONPosition Title: Community LiaisonProgram: Polaris Center General Purpose: Under the direction of the General Director, to coordinate all staff hiring and to process referrals and coordinate admissions to the day and residential programs. Essential Duties and Responsibilities:1.???? Acts as a single point of entry for all day and residential referrals to the Polaris Program including reviewing an assessing Program’s ability to provide appropriate services for each applicant, scheduling and coordinating student/family pre-placement interviews, conducting tours of the facility, assessing child’s current treatment and/or family issues and educational needs, consolidating assessments and interview information and making recommendations to the Placement Review Committee on accepting or rejecting the applicant.? Develops public relations material on new initiatives and services for distribution to referral sources.2.???? Coordinates and manages all aspects of new student admissions such as advising staff of dates/times of entry of new admissions, providing background information, serving as chairperson for admissions meeting with students, family, DCF and internal staff, developing and distributing initial treatment plans to all interested parties.3.???? Recruits, interviews and recommends for hire program staff.4.???? Attends weekly meetings of Polaris management team, serves as Chairperson for statewide Intake Coordinators Committee and Polaris representative to CT Association of Nonprofits Children and Youth Council. Knowledge, Skills and AbilitiesBS/BA in psychology or human services with a MS in sociology desirable plus 3 years of experience in social work or equivalent combination of education and experience.? Must be able to review and comprehend sophisticated psychological and psychiatric evaluations and clinical diagnoses.? Must be thoroughly familiar with referral source (DCF, LEA’s) guidelines.
