Mission Statement

The mission of Capitol Health Network (CHN) is to support and strengthen the healthcare safety net and to advocate for vulnerable people in Sacramento and the surrounding region.

CHN achieves its mission through programs, advocacy and partnerships with health care centers, organizations and individuals that support access to care.

? FQHC Council ? Health center leadership peer network meets monthly to share information and develop collaboration between primary care providers

? Behavioral Health Joint Operating Committee ? Formed in 2014 to meet new ACA benefit ? 5 health centers and El Hogar formed network to develop Integrated Behavioral Health Services ? Next Generation: Clinically Integrated Network

? Patient Registry Work Group ? Formed in 2015 to develop population health management tools ? Focus on PCMH workflows, timely care transitions, gaps in care ? HALO, Health for All and El Hogar lead pilot agencies going live in Fall 2015

? Care Navigation Council ? Forum for navigation service providers to collaborate, share best practices and define standards ? Working with two health centers to develop navigation tools and protocols for accessing appointments and improving care coordination

? SMART Program ? Collaboration between Sacramento County, El Hogar, CHN, Social Security Administration and community providers: provides Social Security benefits advocacy for individuals who are homeless or at-risk

Model of Care: Patient Centered Medical Home, Integrated Care, Strong Focus on Behavioral Health needs and Supportive Services.

Community Engagement: Equally Well community initiative to reduce health disparities

Upcoming Expansion: moving into a new building that will triple capacity for primary care services and double dental capacity.
