Compassion N Care is a home and community-based organization that provides consultation to health care

providers, as well as a wide range of home health services designed to assist patients and their families in returning

to a full and enriched life.

Mission: To provide comprehensive community-based health care solutions to promote independence and wellness

among the disabled and the elderly.

Our services include, but not limited to: Nursing consultation services, home health care, and homemakercompanion.

Nursing Consultation Services: Includes general nursing assessment, independent nurse consultation to review

policies and procedures, staff training, quality assurance and compliance, as well as health care coordination to

ensure better continuum of care for patients and/or as directed by their healthcare practitioners.

Home Health Care: Our home health subsidiary is state licensed, Medicare-Certified, and CHAP accredited. The

services include Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social

Services and Homemaker-Home Health Aide Services.

Homemaker-Companion Services: Includes 24 hour Live-In Care, Companions, Homemakers, Personal Care.
