I, the above signed parent/legal guardian of the minor listed above, grant said minor permission to audition for and if selected, to perform during “Puttin’ on the Ritz” held on Saturday, April 22, 2017, 6:00 p.m.

I also, grant to the Rialto Unified School District, the right to use the above named minors photograph or likeness in original or altered form, with or without the minor’s name, for television broadcasts and other media outlets, including all reproductions thereof.

I acknowledge and consent that parents/guardians cannot be present during any part of the audition, however, a waiting room is provided. I understand performances are to be 3 to 5 minutes (maximum), and the audition may be interrupted due to time constraints. I am aware that the audition judges are unbiased, trained professionals and do not have any children or relatives enrolled as students in the RUS District. I further recognize that participants will be judged according to their age group, originality, presentation, form, technique and appropriate content and that a scoring system will be used to produce fair and accurate results, independent of any biases.
