Annual Performance Report (APR) Instructions

Cover Page Instructions

The Cover Page (signatory page) must have the original signature of the Chief Elected Official or designee. If the local government has multiple grants, there must be a separate document completed for each applicable grant.

Do not complete the box at the bottom that states “NC COMMERCE- RURAL DEVELOPMENT OFFICE USE ONLY”.

Reports are required for all grants that are active at any time in 2020, and closeout documents have not been received by the State by December 31, 2020. If you are unsure about your closeout status, contact the Grants Management Section Chief at (919) 814-4673 or by email valerie.moore@. Main Summary Instructions

Complete the Main Summary by choosing the Activities the grant has by placing a check mark by it. Please be aware that Reconstruction activities, Clearance activities related to Rehabilitation and Temporary Relocation related to Rehabilitation should all be classified under Rehabilitation.

In the boxes to the right, type the amounts budgeted for the entire grant and the requisitions paid between 01/01/2020 and 12/31/2020 in the correct field. If you do not have a budget activity for Planning and/or Administration, leave blank.

Activity Indicator Form and Other Indicators Form Instructions

The Activity Indicator Form uses performance measures indicators. The Other Indicators Form resembles the Accomplishments and Beneficiaries Form used in past APRs. There is only one generic Activity Indicator Form Template and one generic Other Indicators Form Template. Use the generic templates to produce the two required completed forms for each activity in your grant except for Planning and Administration.

There are seven major sections of indicators: Housing Program, Homeownership Program, Economic Development Program, Non-Economic Development Public Facilities Program, Non-Economic Development Public Infrastructure Program, and Public Services Program.

Provide both Proposed (P) (for the entire grant) and Actual (A) only data for each set of indicators for the activity. If a section does not apply, LEAVE BLANK AND DO NOT COMPLETE. For economic development activities, use PFT (Proposed Full Time), PPT (Proposed Part Time), AFT (Actual Full Time), and APT (Actual Part Time).

Please note that Housing Activities under the Housing Program Indicators and separate infrastructure activities under Non-Economic Development Public Infrastructure Program have been broken out as familiar budget line items and to provide additional IDIS information. If any of your approved project activities fall within any of these sections, complete an Activity Indicator Form and Other Indicators Form for each one. Under the Housing Program Section or the Non-Economic Public Infrastructure Section, provide both forms for each activity as appropriate (i.e., rehabilitation or streets).

For example, if you are acquiring and clearing houses, rehabilitating both owner-occupied and rental houses, and paving a street, you will need to complete an Activity Indicator Form and the Other Indicator Form for each of these activities: (1) acquisition, (2) clearance, (3) rehabilitation, and (4) streets. Rehabilitation activities, regardless of type, require only the one set of rehabilitation forms.

Also, please remember that an Infrastructure hook-up is a housing activity, not an infrastructure activity.

Please note that under Race Code information, the indicators are placed in two different categories: Renter and Owner. Fill out the information accordingly. Also, please select households or persons to verify the type of information given. Note: If you are designating the number of households, how many households are in each unit and how many persons per household, use the commenting box that is place at the end of the Other Indicators Form. Additional note: if you are reporting rehabilitation totals, these are reported by housing units not persons. You may report the number of persons per housing unit, but the requirement is for the number of housing units.

Using the Templates:

Adobe Version:

You may use the Adobe version to complete your APR forms by clicking on each page needed and completing the page by tabbing through the fields. You can type in the required fields certain values that pertain to the answers. In many of the fields, fixed values may be selected from a pull-down list.

If you do not have Adobe Reader on your computer, please click on the link at the bottom of the APR Webpage to get a free version. You should be aware that if you do not have Adobe Professional, you will not be able to save your entries. In this case, you will need to complete your work and print it off. Any later changes will result in you needing to complete the entire form again.

If you need to use multiple pages for multiple activities, you will need to complete them separately and print them separately to turn in. If you use the ‘Entire Document’ choice, then you will need to add the extra pages by selecting the needed page as the separate version.

Excel Version:

You may create a new worksheet for an activity as you go, or you may prefer to create all the new worksheets for your activities in your project prior to copying the templates to the new worksheets. You may want to open the Activity Number Worksheet so the new worksheets will be to the left of it. To save a template to a new worksheet, you must first create the worksheet and rename the worksheet tab to that of the activity.

IMPORTANT: Do not save a completed worksheet to the template tabs.

|1.      Right Click on the tab at the bottom of the Excel page that you want to make a copy of. |

|2.      Select “Move or Copy” |

|3.      Check the box “Create a Copy” |

|4.      Select which Tab you want the new worksheet to be in front of (left of). |

|6.       Right Click that new tab you just created and select “Rename”. Enter that new name. |

Submitting your APR to the State:

The hardcopy of the executed cover page, the Activity Indicator Forms and Other Indicators Forms for each of the activities in your grant, and the Grant Contractor Form must be received by the State by 5:00 p.m. by Monday, February 1, 2021. An email confirming receipt will be sent to the email addresses indicated on the cover page. When your APR is approved, and Compliance Reports are approved, you will receive a confirmation letter.
