2. Maximum order: $1,000,000 per order.

3. Minimum order: 12 weeks at 36-40 hours per week. Supplemental Staffing – 1 shift. Supplemental/Per Diem cancellation must be made within 2 hours prior to the start of a shift or a charge for 1 hour is made. However, the government still reserves the right of Termination for Convenience.

4. Geographic Coverage (service area): VA, MD, MS, AL, SC, NC, NY, ME, CA, IL, & Washington DC.

5. Prompt Payment Terms: 1.5% off invoice if paid within 10 days; 1% off invoice if paid within 15 days; otherwise net 30 days.

Shift Differentials:

Weekday Evening Shifts (1500-2300) are an additional 1.5% above the base rate.

Weekday Night Shifts (2300-0700) are an additional 2.5% above the base rate.

Weekend Day Shifts (0700-1500) are an additional 3% above the base rate.

Weekend Evening Shifts (1500-2300) are an additional 3.5% above the base rate.

Weekend Night Shifts (2300-0700) are an additional 5% above the base rate.

Work Week: Sunday – Saturday

Overtime Rate(s): 1.3 times the base hourly rate for hours worked in excess of 8 hours per day or 40 hours per week. In California, a rate of 1.75 times the base hourly rate will apply for hours worked in excess of 12 hours per day. Overtime will be applied in accordance with state and local labor laws.

Holiday Rate(s): 1.5 times the base rate for any holiday recognized by the Federal government.

On-Call Rate(s): $10.00/hr. Provider is to remain within a 30 minute to 30 mile radius of the facility, carry a pager/cell phone/beeper or be available by telephone.

Call Back Rate(s): If a provider is called back to the facility to work, the regular hourly rate will be charged, unless overtime is being accrued. Telephone consultation is included. A 2 hour minimum will apply in the event of call-back.
