When the time is right for you, our health care providers want to help you and your child transition to a health care provider who cares for adults.

In early adolescence we partner with you to help your child take more responsibility for his/her health care.

At age 18, we use an adult model of health care within our clinic. This means that young adult patients are in charge of:

• their own health care decisions;

• scheduling their own visits;

• giving approval for their health care;

• sharing their own health information

We follow state and federal laws. Older youth need to sign a form if they want their parents to be able to get their health information.

Older youth and their parents should discuss the timing of the transition to a new health care provider who cares for adults with their pediatric health care provider. Often, this happens between the ages of 18 and 24, but may be different with young adults with special health care needs and may vary from provider to provider within the pediatric clinic.

Our Walk In Clinic serves patients up to age 21 years.
