Benefits WebsiteBenefits: Latest NewsLink to CMS latest news page to all Plan Administrators Benefits SummaryEmployees of Governors State University receive a variety of State of Illinois, as well as University specific benefits.The State of Illinois benefits are provided by the Group Insurance Act of 1971, which gives the State's Department of Central Management Services (CMS) the authority and responsibility to design, administer, negotiate and/or contract for benefits. Any change in these benefits is decided upon by CMS and becomes effective for all State of Illinois employees. The current version is dated FY2004. For the most up to date rates and contact information, be sure to consult the current annual supplement, the Benefit Choice Options Booklet. For information on the documentation needed for dependent enrollment in benefits, view Dependent Documentation Requirements and Deadlines.The University also offers optional benefits that are supplemental to the State of Illinois benefits. The University benefits are approved by the Board of Trustees and managed by the University. These benefits include:LifeLong Term Disability Retirement Plans In addition to the State and University benefits, eligible employees are required to participate in benefits offered through the State Universities Retirement System. . These benefits include:Retirement Long Term Disability Each year, the University holds an open enrollment period, typically during the month of May, called Benefit Choice. Any change in State or University benefits is implemented at this time and becomes effective the following July 1. Changes to benefits plans can typically be made only during Benefit Choice or when experiencing a qualifying event or change in status. Some plans allow enrollments and changes at any time of the year.Health InsuranceThe State of Illinois' health insurance plan is available to both Governors State University employees and their dependents who are eligible to receive benefits. Proof of dependent eligibility is required, see Dependent Documentation Requirements and Deadlines. The health insurance is a State of Illinois benefit, which requires participation by all full-time employees. Part-time employees may elect to waive health insurance coverage. Full-time employees may elect to opt out of the health, dental, and vision coverage with proof of enrollment in another comprehensive health plan.There are two types of health care plans available: an Indemnity plan called the Quality Care Health Plan (QCHP), and Managed Care plans consisting of a Health Management Organization Plan (HMO), and an Open Access Plan (OAP). Not all Managed Care plans are available in all areas. Check with your benefits department to verify the plans available in your area. If a plan selection is not made within the first 10 days of employment, employees are automatically defaulted into the QCHP. Dependents will not be eligible for coverage until the next Benefit Choice period, or, if experiencing a Qualifying Change of Status. an employee and the employee's spouse are both employees of the University or any other State of Illinois agency, each must be insured individually. Either or both spouse may elect health coverage for dependents; however, the same dependent cannot be enrolled under both spouses for the same type of health, dental or life coverage. The effective date of coverage begins on the first day of employment. Dependent children are covered up to age 19, or up to but not including age 24 if a full-time student. Semiannual re-certification is required for dependents between these ages and at age 19.Enrollment of a newborn child is guaranteed within 60 days of birth, but not automatic. Full-time and part time status employees working at 50% time or above receive term life insurance in an amount equal to their annual salary, at no cost. Up to $50,000 of this life insurance is tax exempt. Any insurance in excess of $50,000 will be taxed as imputed income.Additionally, benefits-eligible employees may choose to apply for and purchase optional life insurance for themselves and/or their eligible dependents through the State of Illinois (Minnesota Life) and University (ING/Reliastar) plans. The cost of optional plans is paid through payroll deduction.Child Life InsuranceThrough State of Illinois (Minnesota Life), dependent children are covered on life insurance up to age 19, or up to but not including age 24 if a full-time student. At age 19, employees must certify the dependent. Thereafter, semiannual re-certification is required for dependents between these ages during the months of October and March. You may apply for life insurance for yourself in any amount between $10,000 and $250,000 in $5,000 increments. Employee life insurance in the amount of $30,000 is guaranteed within the first 30 days of employment. Employee life insurance in amounts over $30,000 may be applied for at any time with evidence of good health. You may apply for spouse life insurance coverage in any amount between $5,000 and $95,000, at any time with evidence of good health. Spouse life insurance may not exceed the amount of employee coverage. You may apply for child life insurance in the amounts of $5,000 or $10,000 for each child. Child life insurance is guaranteed within the first 30 days of employment. Employee and spouse coverage terminates when your insurance age is 70. Active employees beyond this age may continue coverage until the end of the current pay month following the date they stop active work. This life insurance is portable, so if you should leave the University, you have the option of taking your life insurance coverage with you to age 70.
