The SFA's verification notification letter does not include all required information, as follows: The long non discrimination statement. Corrective Action: The FSD and I discussed that the correct long version of the nondiscrimination statement must be on all verification documents sent home to household. 1409. Storage violations observed, on-site, off-site?

Finding 9150: Storage violations were observed. The SFA must ensure that all of its facilities for the handling, storage, and distribution of purchased and donated foods are properly safeguarded against theft, spoilage, and other loss. Please ensure that all items are being date marked as they are being received and then if they are pulled from the original packaging to be storage individually.

Corrective Action: The FSD and reviewer have discussed that date marking is a part of their food safety plan and should be completed at all times. No further action is necessary.
