2014 Verification of CEU (Continuing Educational Unit) Credits

Please sign and complete CEU credit hours earned on the bottom of this page. A copy of this verification of continuing educational unit credits will be forwarded to the IEEE for the issuance of a certificate. Free for IEEE members and $ 35.00 for non-members made payable to the IEEE PES - IAS NY Chapter must accompany this verification and a copy must be retained for your personal records. It confirms that you attended the programs beginning on January 29, 2014 and ending in December 16, 2014. You are responsible for maintaining the record of classes attended. Your attendance will be verified by the sign-in sheets distributed at each course session. This record will determine the number of CEU credit hours earned. Each session qualifies for two (2) credit hours. Form must be submitted no later than February 10, 2015 for credit.

The program was sponsored by the IEEE Power and Energy Society and the Industry Application Society of the New York Chapter. CEU credit certificates will be mailed to you directly by the IEEE upon receipt of a properly completed and executed form (shown below) filled out completely and mailed to the IEEE. We will do the submission to the IEEE upon receipt of your completed form and accompanying check (for non-members only). You must also submit at least one evaluation form for the course, attached.

A CEU credit hour is defined as 60 minutes of continuous participation in a group program. Credit is given only for full credit hours. Each session represents two credit hours. A maximum of twenty four (24) credit hours or 2.4 CEUs can be earned for this program.
