SUBJECT: Chemotherapy Administration

POLICY:

1. When administering chemotherapy, the nurse will follow the appropriate guidelines for agent being administered. This will include:

a. Vascular access considerations.

b. Hazardous waste management.

c. Appropriate attire for safe administration.

2. Continuous infusion vesicant or irritant chemotherapy will be administered via a central line only.

1. The catheter placement shall be verified before any chemotherapy is given by an easy blood aspiration. Once placement is confirmed flush catheter with 10cc of normal saline, connect IV tubing and begin infusion. If you are unable to obtain a blood return refer to policy 2.3.7 (CVC Management Non-Functioning).

2. When the infusion is complete, clamp catheter and disconnect tubing.

3. Attach 10cc syringe of saline and check for a blood return if a positive blood return is obtained then flush line with 10cc of normal saline.

4. If you are unable to obtain a blood return call UICH for further instructions.

5. Appropriate precautions shall be taken by nurse clinicians to protect themselves from accidental contact with the antineoplastic agent. This may include wearing a lab coat, gloves and goggles/glasses when administering chemotherapy.

6. Appropriate procedures shall be used in the event of accidental contact or spill of antineoplastic drug.

7. If at any time during the administration, infiltration or other acute adverse side effects occurs or is suspected, DISCONTINUE THE DRUG ADMINISTRATION AND INITIATE THE PHYSICIAN-DICTATED INTERVENTIONS.

8. All supplies used in the administration that have come in contact with chemotherapy shall be disposed in an appropriate container for cytotoxic drug disposal.

14. Document the following on the Patient Visit Record:

a. Positive blood aspiration prior to and after infusion.

b. Drug, dosage administered and device used to administer.

c. Any adverse reactions

d. Any side effects experienced or discussed

e. Follow up instructions given
