Dr. began the residency in Spinal Cord Medicine at the Rehabilitation Institute of Chicago/Northwestern University Medical School on __________ and successfully completed training on ___________.

This is the Department of Physical Medicine and Rehabilitation’s final and permanent evaluation of Dr. __________ performance during his/her residency. Our training program includes inpatient rotations at the Rehabilitation Institute of Chicago and Northwestern Memorial Hospital in the areas of acute inpatient rehabilitation and acute spine injury care. Non inpatient rotations include inpatient/outpatient therapies and outpatient clinics in orthopaedic surgery, neurological surgery, plastic surgery, urology, orthotics and pediatrics.

Throughout his/her training, Dr. ___________ was evaluated on patient care, medical knowledge, practice based learning and improvement, interpersonal and communication skills, professionalism and systems based practice. Dr. __________ has successfully completed all rotations of his/her training.

Dr. __________ has never displayed signs of any drug or alcohol problems or suffered from any physical or mental health conditions which could affect his/her performance. Throughout his/her training, ______ was not involved in any disciplinary action and complied with the rules and regulations of the medical staff. We are not aware of any personal or professional reasons that would indicate _______ privileges to practice rehabilitative medicine should be limited, postponed or denied.

It is felt that Dr. has attained sufficient professional ability to practice the specialty of Spinal Cord Medicine competently and independently.
