2 JOB PURPOSE

To develop client / family or children and young people (as appropriate) led support using person-centred approaches.

To undertake assessments and to produce and review support and safety plans for individuals to ensure a high quality standard of support that promotes independent living.

To ensure a high standard of service delivery is achieved by championing Home's values, policies and procedures including safeguarding clients / families or children and young people (as appropriate) and other people at risk of abuse.

3 DIMENSIONS Detailed below are the financial measures and statistics relevant to this post.

Co-ordination and development of Level 1 and Level 2 Support Workers. Co-ordination and support of Volunteers, Mentors and hosts, including

befriending and peer support volunteers. Provide induction support and guidance to new colleagues and volunteers. No direct responsibility for a budget. Ensure application of any agreed screening processes, and make eligibility,

referral, allocation and assessment decisions based on risk and client / family or children and young person (as appropriate) need Deputise for Client Services Manager / Senior Client Services Manager Provide direct support to higher risk clients requiring more intensive support in specialist services Co-ordination and lead responsibility for engagement with formal panels and processes such as MARAC, Safeguarding, team around the child/family, MAPPA, and any multi agency health meetings. You are required to be flexible to work within other office/service locations in the future within an agreed area of your initial office location

5 KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED Detailed below are the knowledge, skills and experience required for satisfactory job performance.

Experience/Knowledge

Significant experience of working in a support or care environment is essential Knowledge and experience of relevant client groups to the service. Experience of assessing and monitoring clients' / families or children and young

peoples' needs and skills (as appropriate). Experience of person-centred support and safety planning. Experience of co-ordinating or developing other support workers Experience of inducting, co-ordinating and supporting volunteers (where

applicable). Experience of overseeing landlord and housing management duties, including

re-letting of rooms, collection of rents and reporting repairs (where applicable). Experience of taking formal tenancy action, up-to and including court

proceedings (where applicable). Experience of working as part of a team. Basic knowledge of housing benefit, income support and other welfare benefits

is desirable. Knowledge of issues affecting individuals who require support. Experience of supporting clients / families or children and young people (where

appropriate) to access the resources available to a local community Knowledge of the statutory framework of provision of support for the client group

/ families or young people (as appropriate). Knowledge of SMART principles Basic understanding of housing law i.e. tenancies, evictions etc. Good understanding of relevant regulatory requirements e.g. Care Quality

6 KEY RESULT AREAS The outputs of the job include:

To actively seek to empower clients / families or children and young people (as appropriate) and promote independent living.

To act as the lead case worker for a specified number of clients / families or children and young people (as appropriate). Develop, implement and review support and safety plans, facilitating clients / families or children and young people (as appropriate) to lead the process. Where appropriate, work with client / family or children and young people (as appropriate) advocates and/or representatives from other agencies to co-ordinate an integrated support/care package.

Ensure action is taken to comply with specific contractual requirements of the service in delivery of support to clients / families or children and young people (as appropriate).

Use an asset-based approach to assessment and support planning which recognises the client's / families' or children and young peoples' (as appropriate) strengths, abilities, skills and aspirations and seeks to encourage their development.

To sign-post to relevant agencies and provide support around issues such as independent living, tenancy management, welfare benefits, budgeting, life skills, employment, education, training and resettlement (where applicable).

In line with the support and safety plans and the aims of the service; plan and prepare with clients / families or children and young people (as appropriate) to move to independent living and/or other appropriate support services as required.

Support the client / family or children and young people (as appropriate) to access activities and facilities of their choice within the community in line with their support and safety plan.

Support, enable and empower clients / families or children and young people (as appropriate) to work towards living independently of services.

Provide personal care where agreed in the clients support plan (Care Quality Commission regulated care services only).

To make decisions in relation to referral acceptances and admittances in line with local service procedures.

Facilitate clients / families or children and young people (as appropriate) to access advocacy services and, where they agree, act as an advocate on their behalf in relation to issues identified in their support and safety plan.

To keep up to date with knowledge and own ability to work within the statutory framework relevant to the client group / families or children and young people (as appropriate).

To co-ordinate and develop Level 1 and Level 2 support / care workers where required, reviewing case loads and practice.

To co-ordinate, induct and support volunteers within their Role Descriptions and in line with the Volunteer Policy and Handbook (where applicable to role).

To provide support in specialist areas where identified with the client / families or children and young people (as appropriate) as part of the support and safety plan.

To oversee or administer medication in line with Home Group's policies and procedures.

To implement and adhere to Home Group's policies and procedures:

Perform to all Home Group's standards and policies. To maintain clear and accurate records of work in line with Home Group's

policies and procedures, Statutory Framework and legislation. To implement and adhere to policy and practice in relation to health and

safety. To take responsibility for your own health and safety within the workplace, and

that of colleagues, clients / families or children and young people (as appropriate) and the general public. Ensure that Safeguarding procedures are followed at all times, and to seek advice if necessary from statutory agencies. To directly audit, undertake or oversee housing management duties carried out by the Support Worker i.e. managing rent accounts, tenancy obligations, void works, defect checks as required to ensure client promise standards are being met (where applicable to role). To undertake formal action in breach of tenancy conditions, in consultation with the Senior /Client Services Manager, up to and including court proceedings, where required (where applicable to role). To adhere to and operate within the rent arrears and voids policy and procedures minimising voids and maximising income (where applicable to role). To implement and adhere to policy and practice in relation to property maintenance, including cleaning where required. To adhere to Home Group's Working Professionally Policy and Integrity at Work Policy. To manage risk in accordance with Home Group's policies and procedures. To be responsible for the client / families or young person's (as appropriate) referral and induction process at service level, including; issuing appropriate tenure and other agreements where required. Ensure clients / families or children and young people (as appropriate) understand their rights and responsibilities within the service and as a client / service user of Home Group. Ensure that meaningful involvement of clients / families or children and young people (as appropriate) is central to all activities, applying equality, diversity and anti-discriminatory policies and ensuring rights to privacy and confidentiality. Prepare reports on a monthly basis, compile statistics as required and use, maintain and update project records and systems. Contribute to the development and review of local procedures, with meaningful involvement of clients / families or children and young people (as appropriate) in this process.

To implement and adhere to Home Group's financial procedures.

Receive and accurately record rental income within established Home Group procedures (where applicable to role).

To efficiently operate and record the petty cash system. To support clients / families or young people (as appropriate) to claim welfare

benefits where appropriate e.g. housing benefit. To appropriately support clients / families or young people (as appropriate)

with budgeting skills. To support volunteers to use the relevant IT or paper-based systems to claim

expenses (where applicable to role).

To adhere to, and operate within Home Group's financial policies. To use relevant IT systems to record and input financial information and other

relevant data.
