|JOB SUMMARY |

|The Nurse Case Manager, Liver and Pancreas Center (LPC) is a registered nurse who is accountable for effective coordination of the liver/pancreas disease patient across|

|multiple settings including diagnostic, treatment and follow/up phases of care. The Nurse Case Manager, LPC systematically triages patients based upon clinical |

|problems and educational needs, provides coordination of care for the liver/pancreas disease patient, and is also a resource to clinic staff in regard to the direct |

|provision of care. |

| |

|The Nurse Case Manager, LPC is an active member of the multidisciplinary team and collaborates in the development, execution and evaluation of the multidisciplinary |

|plan of care. He/she considers factors related to ethics, safety, effectiveness and cost in planning and delivering care. |

| |

|The Nurse Case Manager, LPC establishes goals and strategies for meeting the continuing care needs of the patient, family, and/or other care provider. He/she provides |

|care in collaboration with other nursing staff members. He/she works with clinical research personnel to integrate research into patient care. |

| |

|The Nurse Case Manager, LPC utilizes research findings in liver and pancreas diseases and cancers. He/she demonstrates knowledge of and participates in improving |

|organizational performance activities. The Nurse Case Manager acquires and maintains knowledge nursing practice and assumes responsibility for the professional |

|development of self, other nursing staff and students. |

| |

|The Nurse Case Manager, LPC works with the team to assure that systems of care are in place for patients to move across the continuum of care. This may involve clinical|

|areas such as hepatology, hepatobiliary surgery, oncology, and transplant. This includes development of patient care protocols, order sets, and systems that allow care|

|to take place in an effective and efficient manner. Also puts systems in place to evaluate the effectiveness of patient care. |

|MAJOR RESPONSIBILITIES |

|CLINICAL CASE MANAGEMENT: |

| |

|The Nurse Case Manager, LPC facilitates the referral and evaluation of new patients to the Liver Pancreas Center by obtaining, consolidating, and synthesizing patient |

|care records and imaging prior to the appointment, in an effort to optimize the consultation time at UW Health. He/she helps to triage patients to the appropriate |

|discipline(s), for example, hepatology, transplant, hepatobiliary surgery, radiology, and/or oncology. He/she helps coordinate multi-disciplinary care between |

|specialties and clinics. |

| |

|He/she regularly attends, coordinates and documents weekly tumor board meetings pertaining to patients with liver and/or pancreas cancers. He/she attends outreach |

|clinics for patients with liver disease as needed. He/she readily triages patients between locations and other clinics, both at UW Health and at local facilities. |

|He/she provides support to new patients and patients in crisis (recurrent disease, complications), helps to identify learning needs, and participates in patient and |

|family education. |

| |

|The Nurse Case Manager, LPC assists patients and families as they receive care from different specialties by helping them get from clinic to clinic, arranging follow up|

|to ensure smooth transitions in care, ensuring medical information/records are transferred, ensuring consults/procedures with other services are scheduled and occur |

|(i.e. oncology, hepatology, surgery). He/she collects and organizes pertinent outside medical records, pathology slides, and images and load them into PACS for patients|

|referred to UW Health to ensure that their consult will be optimally informative, productive, and efficient, so that patients and families will feel most informed and |

|satisfied with their UW Health consultation. He/she coordinates and schedules care between disciplines at UW Health and at local facilities, and follows up on test or |

|procedure results (such as pathology findings from biopsies) in order to optimize overall care of these patients. He/she will help facilitate communication with |

|referring providers |

|The Nurse Case Manager, LPC facilitates the submission of patient insurance-related paper work. He/she works with the inpatient case manager to facilitate patient |

|transitions before and after hospitalization. He/she incorporates home care planning into the multidisciplinary plan of care on an ongoing basis. |

|He/she integrates nursing interventions into the multidisciplinary plan of care, incorporating appropriate standards of care and practice and patient outcomes. |

| |

|The Nurse Case Manager, LPC implements safe, competent, and efficient patient care within policies, procedures and standards, and interventions as noted on the |

|multidisciplinary plan of care and documents the nursing process to reflect a comprehensive and integrated approach to patient care. He/she evaluates patients’ |

|families’ responses to care based upon the effectiveness of nursing interventions/actions in relationship to established outcomes and consults with Medical Staff, Case |

|Manager, Research Associate/RN, Support Staff i.e. Social Worker, Counselors, etc. and/or Nurse Practitioner regarding clinical variances and recommended changes to the|

|plan of care. |

| |

|The Nurse Case Manager, LPC identifies learning needs of patients/families and teaches by adapting standard information to provide individualized and comprehensive |

|teaching learning. He/she utilizes the resources of the Teaching-Learning Center as appropriate to meet identified patient, family and/or care giver needs. |

| |

|The Nurse Case Manager, LPC performs systematic nursing assessment, follow up on patient outcomes (if they move to another health care system or went south for the |

|winter, notification of deaths), and calls families to offer support after patients deaths. |

| |

|The Nurse Case Manager guides patients’ care by: |

|Interpreting overt and subtle data to determine physiologic or psychological risk. |

| |

|Establishing each patient’s nursing care requirements relative to his/her age specific needs. |

| |

|Establishing goals and strategies for meeting continuing care needs of patient, family and/or other care provider based on ongoing assessments. Involves the patient |

|and family in planning. |

| |

|Teaching the patient, family and/or other care provider critical knowledge and skills necessary to accomplish self-care |

| |

|Managing decision-making of continuing care needs & assures teaching materials are available. |

| |

|Ensuring that patient and family demonstrate knowledge of health status, treatments, symptom management, skills, medications and adaptive behaviors gained as a result |

|of teaching interventions. |

| |

|Ensuring patient and family/caregiver understanding of safety, functional self-care and home maintenance measures gained as result of continuing care planning |

|interventions. |

| |

|Documenting information which leads to insights or the solutions of usual patient problems, the response to and the outcomes of care provided. Documents in accordance |

|with UW Health polices and procedures. |

| |

|Safely and competently executes technical skills required for practice. |

| |

| |

|The Nurse Case Manager, LPC, must demonstrate knowledge of treatment options and clinical trials that are available to the patient with liver/pancreas disease. He/she |

|educates patients regarding the treatment options. He/she demonstrates knowledge of research findings. He/she works with research staff in coordination of patient |

|care, including facilitation of prescription medications and interpretation of symptoms suggesting adverse effects of therapy. |

|He/she collaborates with research associates regarding patient eligibility, patient education, treatment, and monitoring phases. He/she incorporates changes in |

|practice to reflect new advances and ensures effectiveness of interventions. |

|The Nurse Case Manager, LPC demonstrates an awareness of elements in own performance needing further development and seeks opportunities to improve. He/she contributes|

|to the learning experience of students in cooperation with the instructor and other staff. He/she educates peers, patients, and families regarding all elements of the |

|care of persons with liver/pancreas disease or cancer. For example, updates peer and patient education materials regarding the prevention and treatment of side effects|

|of treatments, or in the development of new treatments. He/she identifies and communicates educational needs and shares knowledge gained through participation in |

|continuing education activities. |

| |

|The Nurse Case Manager, LPC maintains appropriate specialty certifications UW Health mandatory education requirements. |

| |

|The Nurse Case Manager, LPC collaborates with the members of multiple different specialties, including hepatology, hepatobiliary surgery, transplant, and oncology, both|

|inside and outside of UW Health. |

| |

|The Nurse Case Manager, LPC, actively participates in quality improvement initiatives. He/she demonstrates positive and progressional written, verbal and nonverbal |

|communication skills. |

| |

|The Nurse Case Manager, LPC applies critical thinking and conflict resolution skills using creative approaches. |

| |
