Goals and Objectives for CNS Practicum

Goal 1: Develop an advanced knowledge and expertise of the Clinical Nurse Specialists role.

1.1 Collaborate with APN’s in order to gain a thorough comprehension of the CNS’s role in the development, management, and treatment of patients with Coronary artery disease (CAD).

1.2 Practice professional leadership skills through the development of the CNS’s role and determine the impact and clinical significance of the position.

1.3 Consult with a CNS on the needs of coronary artery disease and develop guidelines or procedures that will shape my role as an expert in the field of cardiology.

1.4 Prepare and sit for certification as a Critical Care Clinical Nurse Specialists (CCNS) through the American Association of Critical Care Nurses.

Goal 2: Develop advanced knowledge and clinical expertise in the management strategies of CAD patients.

2.1 Actively participate in the direct clinical practice through the assessment, evaluation, and treatment modalities of patients with CAD.

2.2 Review, synthesize, and analyze current literature associated with CAD and implement policies and guidelines based on current research.

2.3 Recommend implementation of standards set forth by the American Heart Disease Association.

2.4 Analyze the local trends that are specific to the community and implement plans to curve and or prevent frequency of cardiovascular disease and readmission to the hospital.

2.5 Attend an annual conference presented by the Cardiology Association Educators and obtain certification provided by the American Heart Association to be an expert in cardiology.

Goal 3: Demonstrate advanced expertise and knowledge of CAD through staff and patient education.

3.1 Create and coordinate quarterly educational in-services for health care professionals on the diagnosis, pathogenesis, and current treatment options for patients diagnosed with CAD.

3.2 Work with preceptor to implement and coordinate a project that will benefit patients with heart disease after discharge from the hospital. This program will help to decrease or prevent incident of readmission to the hospital.

3.3 Collaborate and educate health care professionals on the development and creation of individualized heart disease action care plans to ensure continuity of care is maintained.

3.4 Provide members of the health care team with access to available resources and the latest research on the management and prevention strategies related to CAD. (Including but not limited to the following: Intra-aortic balloon pump, cardiac catherization, Endovascular therapy for aortic emergencies, Management of advanced heart failure, Alcohol and the heart, EKG and interpretations, Cardiovascular and rheumatologic disorders).

3.5 Support and mentor nursing staff on the proper techniques of treatments and the use of adjuncts in the delivery of heart disease medications.

Goal 4: Develop advanced knowledge and expertise in the direct patient-care techniques that are unique to the functions of the CNS’s role.

4.1 Develop and maintain therapeutic relationships with patients utilizing

the American Association of Critical Care Nurses’ Synergy model.

4.2 Institute open communication methods regarding health problems, goals, management, and therapy techniques tailored toward patients needs.

4.3 Support and tailor the needs of each patient regarding the CAD management teaching course by ensuring cultural beliefs and ethnocultural practices are incorporated into each program.

Goal 5: Develop advanced knowledge and clinical expertise of the CNSS’s role in the ethical decision making that is individualized for each patient.

5.1 Implement an ethical-decision making processes that is individualized for each patient diagnosed with CAD.

5.2 Participate in the hospital ethics committee meetings.

5.3 Collaborate with interdisciplinary team in the identification, deliberation and resolution of difficult moral issues faced by the hospital ethics committee.

Goal 6: Develop advanced knowledge and expertise of the CNS’s role in professional leadership.

6.1 Collaborate with APN to gain professional leadership skills in ability to negotiate, consult and problem solve.

6.2 Attend hospital departmental head monthly meetings.

6.3 Support and educate nursing staff through collaboration, negotiation and consultation.

6.4 Practice professional leadership skills through the development of the CNS role to plan appropriate nursing intervention for positive patient outcome.
