August 29, 2019Dear Parents,It is once again time to get this season’s flu vaccine. The Monroe County Public Health Clinic is working with your child’s school to give children the flu vaccine at school so you won’t need to miss work. If your child does not have insurance, has Medicaid, or has insurance that doesn’t cover immunizations, there is not a charge for the vaccine. If your child has insurance that covers vaccines at any level, we are required to bill the insurance company. We are able to bill most insurance companies. Please see the attached list of accepted insurance companies. Flu vaccinations will be offered at Bloomington South High School on November 21st. If you would like your child to be vaccinated at school, please: Completely fill out the attached consent and health questionnaire on the back. Be sure to include you insurance ID number if applicable.Return consent form to your school by November 7th.
