Failure to do so may subject Bidder to disqualification.Please submit the following Bid Form with your proposal.Bellflower Unified School District is reissuing Addendum# 2 to clarify the services being requested for RFP# 19/20-02 E-RATE 2020-2021 YEAR 23 – CATEGORY 1 WAN-ISP is for Lite or Dark Fiber services, corrections corresponding to these updates are on pages 3, 7, and 21 (see attachments of revisions). The District currently leases dark fiber solution from end to end. Bellflower Unified School District is requesting proposals from qualified vendors to provide high-speed lease lit or dark fiber multi-gigabit circuits for the Bellflower USD. The pricing sheet has also been updated to accommodate this change (see attachment). Additionally, attention service providers – a second Mandatory job walk will be held on Friday December 20, 2019 at 10:30 am to cover schools reviewed on previous job walk: District Office, Alondra Annex, Maintenance & Transportation, Nutrition Center, Bellflower High School, Ernie Pyle, Frank?E. Woodruff, and Washington, Albert Baxter, Craig Williams, Thomas Jefferson, Esther Lindstrom, Mayfair High School, Stephen Foster, and Intensive Learning Center and Las Flores. Vendors who have attended previous mandatory job walk on December 11th-12th, 2019 will not be required to attend this job walk. Bellflower USD, is also issuing a revised time line which listed below. Correction to previous question asked in addendum #1:Question: Regarding the Lit dark fiber, do you want the service provider to monitor the network? Is circuit monitoring required as part of the RFP? Correction to response (Addendum #2): Network monitoring is not necessary?for dark fiber services.
