RA-6 GUARANTOR OF LEASE

“RELEASE OF RESPONSIBILITY”

Form RA-6 is used to release a “Guarantor of Lease” from the position of being responsible for paying a Resident's rent and other obligations as set forth in the RA-5 “Guarantor of Lease Addendum” or the Market and non-subsidized Housing Credit Lease (RA-1m) Co-Signer clause.

Place this “Release” with the Rental Agreement in the Resident's file.

The “Release of Responsibility” may be signed under any one of the following circumstances:

1. The resident(s) has/have had twelve (12) consecutive months of on-time rental payments.

2. The eligibility requirements, as set forth in the “Resident Selection Policy” are met by the resident(s) alone.

3. Upon move-out by the resident(s), all charges including but not limited to

rent, damages and late fees are paid in full.

Please complete the form as follows:

1. Enter the DATE the “Release” will become effective, the date of the ORIGINAL RA-5 “Addendum”, the Guarantor(s) name(s), the property name and the unit number.

2. Ensure that the resident(s), Guarantor(s) and Owner/Owner’s Agent sign and date the form.

3. Distribute as listed on the form.
