| |As patients who have been shielding start to get out and about, | | | | |

| |we will be arranging special first appointments for these | | | | |

|6. |48 Hour Call Back | | |10. |Urine Samples |

| |If you call the surgery and request assistance from a GP/ANP, | | | |This is a reminder that all urine samples must be handed into |

| |then you will be told that this may take up to 48 hours, so | | | |the practice by 12 noon with a form completed stating the |

| |those >75 and older who live alone or in sheltered housing who | | | | |

| |may be socially isolated, lonely and would enjoy a regular | | | | |

| |friendly chat over the telephone, however, the service considers| | | | |

| |all applications on a case-by-case basis, so if your | | | | |

| |circumstances are different to this and you feel you’d like a | | | | |

| |call companion, please get in touch. | | | | |
