POSITION SUMMARY

The Stroke Program Coordinator will be a recognized expert in the assessment, treatment and care for stroke patients with knowledge of the special needs of patients with cerebrovascular disorders. This individual will be responsible for managing stroke care services throughout Providence Health & Services Alaska and and will work collaboratively with the Director of the Neuroscience Service at Providence Alaska Medical Center. Responsibilities include developing, implementing, and monitoring evidence-based standards of care for ischemic and hemorrhagic stroke patients, coordination of interdisciplinary team overseeing the care of stroke patients, coordinating and developing patient education materials that address the needs of stroke patients in Alaska, analyzing data related to quality measures, addressing outcomes in this patient population and opportunities for performance improvement, identification of staff education needs and implementation of appropriate programs to address those needs, ongoing management of the telestroke consult system as the liaison from the HUB in the established HUB and Spoke system. Performs all duties reflecting the mission and vision of the organization.

ESSENTIAL JOB FUNCTIONS:

A. JOB DUTIES: (For performance review, assess competence for each essential function using “C” for competent and “NI” for needs improvement)

1. HUMAN RESOURCES: Maintains attendance within policy guidelines; attends educational offerings and assists with the provision of education within the hospital. Completes all tasks and duties accurately and timely. Demonstrates initiative and ability to plan, work and perform job duties autonomously. Assures competency of staff members is assessed, maintained, and improved.

1. PRODUCTIVITY & FINANCIAL RESOURCES: Assists in the preparation of annual operating and capitol budgets for comprehensive services provided by the Advanced Primary Stroke Center . Develops marketing strategies and outreach programs for liaisons and community hospitals. Analyzes program expenses and adjusts according to budget. Identifies additional available resources to promote outreach stroke education in the community and develop a center of excellence in the South Central Region of Alaska. Participates in state wide activities as a recognized resource and expert in the care of the stroke patient.

1. Develops, updates, and maintains protocols and standing orders for all departments involved in the care of the stroke patient.

1. CONTINUOUS PERFORMANCE IMPROVEMENT: Develops and implement’s program performance improvement activities in accordance with hospital and the Joint Commission’s standards for the care of the stroke patient. Ensures that department performance improvement process is consistent with the hospital’s overall performance improvement objectives as well as the mission, vision and values of the hospital. Monitors quality indicators for all stroke patients. Correlates and evaluates patient data pertinent to stroke delivery system. Provides monthly review of stroke cases with reports to Neuroscience Director and Manager of in-patient neurologic areas. Provides written feedback and suggestions for improvement to staff members, physicians, and departments involved with the care of the stroke patient. Monitors use of telemedicine system for spoke hospitals and provides quantitative reports depicting use, outcomes, and service provided to each hospital

1. TEAM WORK: Uses independent judgment to assign work to team members and to responsibly direct team members. Role models collaboration and partnership with physicians, primary team members and other departments. Maintains communication with all area pre-hospital agencies and referring hospitals. Provides education, outreach programs and feedback for pre-hospital agencies. Collaborates with leadership and hospital administration to ensure consistency in planning and designing servicers, directing services, integrating and coordinating services and improving organizational performance in department.

1. PLANNING: Organizes and provides stroke orientation programs as well as ongoing staff development to staff in ACC, PCU, 5N, 3W, and 5W. Provides opportunities for community service and outreach education. Coordinates stroke initiative efforts with EMS agencies and encourages staff to become involved in increasing the ability of rural health workers to provide excellent pre-hospital care. Is the coordinator and primary liaison from Providence Alaska Medical Center for the Telestroke system providing stroke consults to identified ‘spoke’ hospitals throughout Alaska.

1. Implements care/services that recognize age/diversity specific needs/issues of customers served. Develops and oversees implementation of community education programs related to stroke prevention, screening and care.

D. ESSENTIAL JOB QUALIFICATIONS: (Any equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

1. Education: Master’s degree in clinical nurse specialist program accredited by the Commission on the Collegiate of Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC) or Master’s degree completion from an accredited Nurse Practitioner Program.

1. Experience: Minimum of three (3) years experience in neuroscience care with an emphasis on stroke care and neurology preferred. Acute medical or rehabilitation nursing preferred. Demonstrated experience with program development preferred.

2. Licensure/Certification: Current license as a registered nurse in the State of Alaska. Cetification in Neuroscience (CNRN) is preferred. BLS – Healthcare provider; ACLS – obtained within 6 months of hire. Certified as a Clinical Nurse Specialist or Advanced Practice Nurse required.

1. Other Qualifications: Must have knowledge of requirements to maintain Stroke Center Certification and to maintain recognition as a comprehensive stroke center. Must have working knowledge of anatomy and physiology of the Central Nervous System. Must possess leadership skills, clinical expertise in stroke care and excellent interpersonal skills so as to perform in the role of the clinical role model, education, liaison and consultant. Must have knowledge of critical care and emergency equipment and able to use information and computer systems. Must have some knowledge of the financial and operational aspects of health care.
