With reference to the press notification published in the Daily News/Dinamina /Dinakarana Newspaper on 17-05-2021 for the above purpose.

Due to the current covid-19 pandemic situation in the country CCF ltd has decided to issue the above Application form for the registration through F web site .lk.

This online facility will be available to all the applicants who are eligibile to apply for the registration of suppliers for the year 2021/22.

Please be informed the set of Application form together with the general terms and conditions could be downloaded from above web site. .lk.

The duly filled application form along with the General Terms and Conditions and the required mandatory documents should be submitted after the payment of Rs 2000/- a non-refundable application fee to our bank account details given below. The Bank slip should also be submitted together with the application form.

To Colombo Commercial Fertilizers Ltd., (CCF) for the period of 2021/22

01. In accordance with the decision of the Government of Sri Lanka towards the development and use of organic input agriculture in the country, the Chairman on behalf of the Colombo Commercial Fertilizers Ltd. (CCF Ltd), invites eligible Local and International suppliers to submit applications to register for the supply of the following type of organic fertilizers to CCF Ltd for the period of 2021/22.

? Natural Organic compost material with proven Microbial Inoculant (If any) ? NPK Compound (Varied nutrient formulas ? Organic base) ? Any Other similar allied Organic base products

Eligible Local Applicants who have been registered with NFS/ Makandura Organic Fertilizer Manufacturing & Research Institute for the manufacture & supply organic fertilizer under the approved subsidy programme of the Government/ Treasury will be included in a list of registered Suppliers for the year 2021/22 subsequent to an evaluation and such suppliers who are entitled to submit bids. Bids will be invited from time to time during the period of 2021/22, for the supply of above organic Compost fertilizer products and allied products commencing from the date of registration.

Application for the registration will be issued from 17th May 2021 up to 30th June 2021 by the Procurement & Supplies Division, Colombo Commercial Fertilizers Ltd, Hunupititya, Wattala, Sri Lanka. INSTRUCTIONS TO APPLICANTS

Invitation for registration for the supply of Organic Fertilizers and Allied products. Colombo Commercial Fertilizers Ltd., (CCF) for the period of 2021/22

01. The Chairman, on behalf of the Colombo Commercial Fertilizers Ltd., invites eligible Local Internationalsuppliers to submit applications for the registration for the supply of following type of organic products for the period year 2021/22.

? Natural Organic compost material with proven Microbial Inoculant ? NPK Compound (Varied nutrient formulas ? Organic base) ? Any Other similar allied Organic base products 2. The validity period acceptable for the registration of above supply shall be continued up to 31.12.2022.

3. Application 3.1 Applications should be submitted in the form at annexure 1(a) and 1(b) hereto in duplicate (01 original + 01 duplicate). An applicant may apply for one or more types of above products, based on the capacity to supply.

3.2 All information required to be submitted in the Application form should be duly completed and furnished.Incomplete applications which are not complied with the Clause No 5.1 will be rejected.

3.3 The Application Form should be duly signed by the applicant or authorized officer (his/her original signature) indicating his or/her name and the company stamp should be affixed. Any alterations made to the information provided in the Application Form should be confirmed with the initial of the applicant or the duly authorized officer.

4. Eligibility

4.1 Applicants should satisfy the following mandatory requirements.

i.

Should have been registered as a business entity with the Registrar of Business or

appropriate authority in the respective Country for a minimum period of two (02) years

at the time of submission of the application.

ii.

Should have registered with NFS/ Makandura Organic Fertilizer Manufacturing &

Resource Institute as Manufacturer & Supplies of organic fertilizers under the approved

subsidy programme of Government/ Treasury.

iii.

Should have valid Environment permit for the production of compost fertilizer

iv. Should have Local Agent for the International Suppliers, The Power of Attorney should be submitted for this purpose.

5. Documents to be submitted 5.1 The following documents shall be submitted along with each application, in duplicate. The original and Duplicate documents should be kept in two separate sealed envelopes and submitted in one

large sealed Envelope addressed to the Chairman Department Procurement Committee (DPC)

should be written Application for registration of supplying Organic Fertilizers Products and allied products to Colombo Commercial Fertilizers Ltd., (CCF) for the period of 2021/22

a. A certified copy of the Company Registration or Business Registration of the applicant. (See Clause 5.2 below)

b. Copy of Company / Business profile.

c. A certified copy of Audited financial statements including certified copy of the Income Statement for the immediate preceding year. (See Clause 5.2 below).

d. Details of the past experiences in supplying organic fertilizer, and allied products to any organization within Sri Lanka. / Prevailing distribution network and Business proposals.

e. Business Registration should be submitted together with Form 20 confirming the name of the Board of Directors of the company.

f. Certified copy of Power of Attorney issued by Principal Supplier infavour of Local Agent

g. Location and place and details/ Address of the production centers Institutions.

h. Category of the organic products and available capacity of product and raw material used and composition of the product.

i. Quantity and Specification of the product could be supplied per month.

j. Cost of production per MT each unit.

k. Categories of the crops your products could be used and application methods to be used with specific recommendations.

l. Briefly explain the method of Quality Control

5.2 All documents required to be submitted shall be in Sinhala, Tamil, English Languages.

5.3 Each page of all mandatory documents should be certified with the signature and frank of the applicant.

5.4 In the event of any additional information is required applicants shall provide such information immediately.

6. For registration, the duly filled Application Forms should be submitted in duplicate (01 Original + 01 duplicate) in separate sealed envelopes to reach ,The Chairman, Department Procurement Committee (DPC), on behalf of Colombo Commercial Fertilizers Ltd., Head Office, Hunupitiya, Wattala Sri Lanka either by hand delivery or registered post. The sealed envelope should be clearly marked "Application for the registration for the supply of Organic Based Compost product to CCF Ltd for year 2021/23". For the initiating of registration process, the duly filled application together with the Mandatory documents should be delivered to the Procurement & Supplies Division, Colombo Commercial Fertilizers Ltd., Hunupitiya, Wattala on any working day between 9.00 a.m. to 4.00 p.m. on or before 30.06.2021

7. Application could be obtained throughout the period of 2021/22. This system of rolling application is maintained throughout the years valid until 31.12.2022. Eligibility/ineligibility of the applicants as supplier will be acknowledged within two weeks. The successful Applicants those who have paid the non-refundable bidding document fee mentioned in the paragraph 8, will be registered as a supplier for the purpose of supplying Organic based Compost and allied products to the Colombo Commercial Fertilizers Ltd.

8. After the evaluation the successful applicants who satisfy the eligibility criteria as per Clause No.04 will be listed as a preregistered supplier upon a payment of non-refundable bidding document fee of Rs.2,000/= in favour of the, Colombo Commercial Fertilizers Ltd, Hunupitiya, Wattala, Sri Lanka. Consequent to the payment of non-refundable bidding document fee mentioned above the standard bidding documents for the period of 2021/22 will be issued to such registered supplier. 9. Based on the requirement of fertilizers the registered suppliers will be invited bids, time to time as and when necessary by CCF Ltd. 10. All communication with the Applicant/ Registered Supplier will be made by way of ordinary post/ registered post/facsimile message or e-mail to the address, contact numbers and email addresses given in the application.
