Scholarship of Application (shaping public policy on health care)

1. Chair, Medicaid Reform Task Force for the State of Colorado- Colorado Medical Society/Specialty Societies 2006 – 2012

In 2006, an article was published describing how health care provided for Medicaid patients through North Carolina Medicaid CCNC (Community Care of North Carolina) saved the state $200 Million to $400 Million a year. This was done by dividing the state into regions, with regional control, providing primary care through a medical home, and providing active case management for high cost, high risk diseases (beginning with Diabetes and Asthma). At that time, I was President-Elect of the Colorado Academy of Family Physicians and on the Congress for Healthcare Reform for the Colorado Medical Society (CMS). We thought that this model would be good for Colorado and created the CMS/Specialty Society Medicaid Reform Task Force, of which I was the Chair, to try to persuade the State to consider reforming our Medicaid system to this model. We brought together legislators, representatives from CMS, the Specialty Societies (mainly primary care, emergency medicine and psychiatry), and representatives from Health Care Policy and Finance (HCPF), and eventually the Medicaid Director and a number of his associates. We brought in Alan Dobson, MD, who helped to create the system in North Carolina, to meet with HCPF representatives. Over time and with guidance from the Medicaid Reform Task Force, the State of Colorado decided to implement this type of system in Colorado, implementing the Accountable Care Collaborative (ACC) pilot. In its first year from 2011 to 2012, according to Health Care Policy and Finance, “This effort has so far reduced health costs by $20 million and returned $3 million to taxpayers and is expected to save $86 million.” The pilot was so successful that they have decided to implement this system statewide for all of the Medicaid system.

The Colorado Commission on Family Medicine (COFM) held a strategic planning session in the summer of 2010. It was felt that changes are needed to our currently flawed GME funding system to help our family medicine residency programs be in a better position to provide the trained primary care physician workforce that the country needs. A health system with a strong primary care workforce has been shown to reduce costs, while at the same time improve quality and outcomes. There is wide disparity in the amount of GME funding a residency receives across the country, with the Midwest and West being relatively more disadvantaged, and this amount does not correlate with the cost of running a residency. Also, payments are tied to the percentage of Medicare that a sponsoring hospital cares for, and this does not correlate with the cost of running a residency either. Our family medicine residencies largely care for indigent patients, making patient revenue a poor way to increase revenue for a program in order to afford to appropriately run the program, including paying competitive faculty salaries. We met with Senator Mark Udall who recommended that we form a coalition of family medicine leaders from the West and Midwest to discuss this and make recommendations. Through many of my national positions described above, I had a number of connections with national family medicine leaders and we were able to put together such a coalition. After review of the literature, and a number of strategic planning meetings, we had a face-to-face meeting with the coalition (called the GME Initiative) on July 8, 2010 in Denver, Colorado. We came up with recommendations that were not constrained by current policy of parent organizations, and wrote a position paper of which I was a principle co-author. This was shared with the legislators, whom our members had relationships. In response to our position paper and discussions, seven bipartisan United States Senators (four of whom were on the Senate Finance Committee), wrote a letter to the Institute of Medicine (IOM) asking that they conduct a study of the workforce needs of this country, and in particular the primary care needs of the country, and how GME funding can and should help to achieve what the country needs. The IOM agreed to conduct this study, with funding by the Josiah Macy Jr. Foundation. We have provided testimony about this to the IOM. We have written our recommendations in a peer reviewed paper that has been published, in which I was the lead author. Through discussions with Health Advisors to a number of Senators and Congressmen we are planning a GME Summit in Washington to discuss this further. We have already been approached by several different members of Congress to review draft legislation that deals with GME funding and primary care workforce reform.

Medicaid Reform Task Force

The Medicaid Reform Task Force received a grant from The Colorado Health Foundation for $122,000 to fund the work of the Colorado Medical Society Foundation Task Force. In conjunction with State, the Colorado Medical Society, the Specialty Societies, and the Colorado Hospital Association joined together to work on reforming Medicaid for the State of Colorado. I chaired the Medicaid Reform Task Force and lead the initiative from 2006 – 2012.

GME Initiative

I chaired the Strategic Planning Committee, was on Steering Committee, major author of Position Paper, and senior author of peer-reviewed paper exploring needs and alternatives for GME funding to promote workforce needs of the country and promote primary care. COPIC Foundation provided $10,000 grant, 2010.

Rural Training Track Bill (SB13-264)

I was very involved with development and passage of a bill in the Colorado Legislature which provides $500,000 per year from the state, to be matched by federal dollars to make $1,000,000 per year for up to 3 years if money is available in the state budget in subsequent years. This is to support the development of Family Medicine Residency rural training tracks in Colorado. Bill passed in 2013 with funding 2014 – 2016

Rural Training Track Bill (SB14-144)

Helped with the development and passage of bill in Colorado Legislature. Changed the previous year’s bill to $1.5 Million from the state with $1.5 Million federal matching, for a total of $3 Million yearly, for development and maintenance of rural training tracks, and removed the 3 year limit, so that the funding is ongoing. Added a new family medicine resident to 5 programs, $2.7 Million per year, divided between state and federal, and $270,000 per year for faculty loan repayment.
