Award/Service Commitment

Up to $15,000 or $25,000/yr dependent on HPSA score

Three year commitment

Additional funding may be available after initial three year commitment

Up to $25,000 for DDS; up to $6,000 for RDH

Support Staff

Funding through Kaiser Permanente Community Health Fund

Administered by Colorado Community Health Network (CCHN)

Registered Nurses, Advanced Practice Nurses, Nurse Faculty

Must work full-time (32 hours/week)

Clinical support staff with a Bachelor's Degree or less

Must have worked at their current clinic for a least six months

Located in a HPSA For Nurse Faculty: any

accredited public or private not-for-profit school in the US Other Nurses: located at a Critical Shortage Facility

Community Health Centers, ClinicNet Safety Net Clinics, or Rural Health Clinics in Colorado

Located in a HPSA Accept public insurance

and have a sliding fee scale for low income patients

Up to 60% of unpaid student loans
