PROPOSAL PART IIISCOPE OF WORK (“SOW”)ObjectiveThe Community Impact Activities Management provider will manage the study activities as directed by the HCS County Coalition. This proposal is to manage activities in one or more of our Wave 1 counties (Cayuga, Columbia, Erie, Greene, Lewis, Putnam, Suffolk and Ulster) beginning on April 1, 2021 through December 31, 2022. This management service will include disbursement of funds to selected subcontracted partners as designated by the HCS County Coalition, as well as fiscal management, oversight, and reporting. The overarching objective of the HEALing Communities Study (HCS) is to test the impact of a comprehensive, data-driven community response plan to reduce opioid-related deaths. There are 16 counties in New York State participating in two study waves with each county required to establish an HCS Coalition. The HCS County Coalition is charged with analysis of data and local culture; selection of evidence-based practices (EBPs) to increase opioid overdose education and Naloxone distribution, medication for opioid use disorder and safe prescribing practices for opioid; creation and approval of a county-specific action plan; implementation and monitoring of the action plan; and funding for community impact activities to achieve study goals. Some counties participating in HCS require contracted support to contract with community organizations and/or providers for community impact activities in an expedited timeframe as required to achieve HCS milestones. As a support to county partners, Columbia Social Intervention Group (SIG) will contract with a “Community Impact Activities Management” entity for a participating County. This Community Impact Activities Management provider will act as an independent contractor responsible for the defined munity Impact Activities Management The role of the Community Impact Activities Management entity will be:Work with and report to the HCS County Staff on available HCS community impact funds.Receive the approved HCS Action Plan with authorized fund allocations, scopes of work, deliverables, and timeframes for each community impact fund recipient.Subcontract with county-selected community impact fund recipients for the items and terms provided by the HCS Coalition, subject to pre-approval by Columbia. Monitor financial and scope of work performance quarterly.
