I understand that I am releasing the school from the responsibility of transporting my child home from said event. I verify that I will only be transporting my child/children and no one else.

THIS FORM CAN ONLY BE SIGNED/APPROVED BY THE H.S. PRINCIPAL, THE SUPERINTENDENT, OR THE ATHLETIC DIRECTOR. WITHOUT THIS FORM ATHLETES MAY BE DENIED PERMISSION TO LEAVE EARLY FROM A CONTEST.
