Grad Student Advising Form - Columbus State University

Grad Student Advising Form – Spring 2014 (Security Certificates)

Please fill out the top of this advising form, read the General Guidelines section very carefully, and then indicate your Spring 2014 course selection (and articulate whatever questions or concerns that you may have) in the space provided at the bottom of this form.

This form is to be filled out by those CSU students who are pursuing one or both of the following two security certificates:

- Graduate Certificate for Information Systems Security Professional

- Graduate Certificate for Information Systems Security Officer

If you are going for an MS in Applied Computer Science, perhaps in addition to one or both of the above certificates, please do not fill out this form. Instead drop me a line (by sending an email to chouchane@columbusstate.edu) so you and I can get your advising done for your MS.

MS and certificate students may use email at any time to schedule an appointment with me for a face to face meeting or a phone conversation.

Today's Date: ___________________________

Your Full Name: ___________________________

Your Student ID (i.e., your 909 number): ___________________________

Your School Email Address: _______________@columbusstate.edu

Is your current admission status Provisional?(Yes/No) ________

If your current admission status is Provisional, what requirements does your admission letter/email say you still have to satisfy (CPSC 6105? CPSC 6106? GRE? Other?) to switch to Regular status? (Do not include the requirements that you have already satisfied). If your current admission status is not Provisional, go to the next question. If your answer is 'Other', please elaborate.

In order to switch to Regular status I have to satisfy the following requirements: __________________________________

When did you take the first graduate CPSC course that you are looking to apply towards your certificate? This course was taken in the _________semester of the year _______________.

What is the date at which you expect to finish the coursework for your certificate? My expected certificate date is: ______________________________

The security certificates (Information Security Officer or Information Security Professional) that are offered by the School are described at . What security certificate(s) are you interested in going for: ______________________________

General Guidelines: Read the following guidelines very carefully

1) Financial Issues: Refund policies, payment policies, and important deadlines are determined by CSU's administrators. Current information regarding this is available at . Answers to your Frequently Asked Financial Aid questions are available at .

2) Applying for your certificate: A month or so before the last day of your last certificate class, please send me an email to confirm that you are going to apply for your security certificate(s). After you receive my confirmation that you may go ahead and apply for your security certificate(e), send an email to cs@columbusstate.edu and to the Chairperson of the TSYS School of Computer Science. This email should CC me, and should list the classes that you have already taken, as well as the class(es) that you are currently taking, and should request that you be awarded your security certificate(s) (Information Security Officer and/or Information Security Professional) if your coursework requirements for the certificate(s) will have been satisfied once your grades for the current semester are in.

3) To learn about CSU's important Spring 2014 deadlines (such as the deadline to withdraw with no financial penalty and the deadline to withdraw with no academic penalty), study CSU's Academic Calendar (). The full official Academic Catalog for 2013-2014 is available at .

4) Graduate students who work full time are advised to take no more than one or two courses per semester. Students must choose their course load very carefully so that they can comfortably handle their schoolwork while working full time.

5) Students whose admission status is Provisional must make it a priority to switch to Regular status. This is done by satisfying the requirements outlined in their admission letter/email. (e.g., take the GRE, take CPSC 6105, and/or take CPSC 6106, etc.)

6) CPSC 6105 and CPSC 6106 are foundation courses and do not count towards the 36 hours of required coursework that graduate students must complete to satisfy one of the requirements for graduation with the MS. CPSC 6107 is not a foundation course, and may count towards the 36 hours of required coursework that graduate students must complete to satisfy one of the requirements for graduation with the MS.

7) Advisees must consult with me well before registering for courses and well before attempting to drop a course, even when no advising hold appears on the student's record and prevents them from registering or dropping a course. It is ultimately the student's responsibility to choose their courses and their course load and make sure they can comfortably handle the load and the study hours. Never take any action (such as registering for a class or dropping one) that may have any effect on your progress towards your certificate or certificate award date without first consulting with me.

8) Like all graduate students, certificate students who withdraw from a graduate course after the “official deadline to withdraw from a course with a grade of WPASS” are assigned a grade of WFAIL by the system. A grade of WFAIL will be used by the system like an F grade in the computation of the student's cumulative GPA. Graduate students must maintain a cumulative GPA above 3.0, and never earn a C or below in two or more classes, or they risk being placed on Academic Probation or Academic Exclusion (). There is no grade forgiveness policy for graduate courses. If a student makes a WF in a given graduate course, he/she may certainly retake the course, but the WF will still be used in the student's cumulative GPA, even after the course with a WF is retaken and a new grade in the course is obtained. This non-forgiveness rule applies to all GPA-affecting letter grades that a student may earn in a graduate class (including CPSC 6105 and CPSC 6106). Grades (including WF) may be appealed, however, if certain conditions are satisfied. See the Office of the Registrar's FAQ section () for more on this and for answers to other questions typically asked by students.

9) Now study the MS Course Requirements page (at ) and the MS graduation Requirements (at ).

11) Now study the class schedule for Spring 2014 (). Classes that are at the 5000U level are undergraduate classes and cannot be used for graduate credit. If you have taken the U version of a 5000-level course (e.g., CPSC 5157U), then you may take the G version of the course (e.g., CPSC 5157G), but you will not be able to use the G version of the course for graduate credit.

12) Study the School's Course Description page at (). Note: If a 5000-level course has an undergraduate course as a prerequisite (for example, CPSC 5157 currently has CPSC 2108 as a prerequisite) then you will not have to first clear the prerequisite before you can consider taking the G version of the course (for, example CPSC 5157G), that is unless you are an undergraduate student.

What classes are you looking to take in the Spring of 2014? Please give the name (e.g., Computer Networks) and number (e.g., CPSC 5157G) of the one or more classes that you want to take in the Spring of 2014. You may list your class selection in order of preference, regardless of whether or not you would like to take them all.
