COMMUNITY HEALTH WORKER – ADOLESCENT SPECIALIST

FLSA Status: Non- Exempt

POSITION SUMMARY

The Community Health Worker (CHW) - Adolescent Specialist will work with adolescent/teen parents within the Pittsburgh Public School (PPS) Teen ELECT program, as well as other teen parents. In this role, the CHW will execute the Healthy Start model via in-school and in-home educational and care coordination services to promote school engagement, prevent student’s displacement from their family home, support positive pregnancy and childbirth outcomes, identify and treat/refer for depression, provide parenting education using the Nurturing Parenting curriculum and support the development of life skills. This is an advanced level community-based position that requires strong interpersonal skills, understanding of child and maternal health and social work concepts, the ability to work independently and the ability to exercise sound judgement.

Essential Duties and Responsibilities

• Provide school-based and in-home health education to Teen ELECT students/Healthy Start program participants and connect participants to other community resources.

• Collaborate as a member of the HS/PPS project team to continually evaluate student needs and assess program progress.

• Complete a comprehensive assessment based on the perinatal period of risk and assist participant in developing a plan of care.

• Assist students in mitigating and/or resolving family conflict by providing goal planning and care coordination.

• Deliver training and coaching to PPS staff, when needed.

• Possess specialized knowledge in working with vulnerable populations, i.e. adolescents, participants with CYF involvement, who are experiencing homelessness, who are experiencing trauma and family conflict, etc.

• Identify the social determinants of health affecting the participant and develop a plan to address those issues.

• Obtain information, such as physical, psychological, and social factors contributing to the participant’s situation and problem, and evaluating that information to determine needs.

• Deliver direct casework services to participants, including follow-up and referral to appropriate resources.

• Consult with multidisciplinary team on participant care plan, needs, and goals during case review.

• Maintain accurate, professional and up to date participant records, correspondence and reporting.

• Performs other duties or tasks as directed.

Qualifications

Must have a passion for working with teens and teen parents, with demonstrable experience working successfully with vulnerable populations. Bachelor degree in the field of social work, maternal child health or family related studies preferred. Combinations of education and experience will be considered.

Skills and Knowledge Requirements:

• Passion and high levels of energy needed to engage teen population

• Ability to work effectively in a school environment and travel between PPS sites

• Understanding of child and maternal health and the Healthy Start program

• Effective verbal and listening communication skills

• Ability to respectfully communicate with persons from various education and cultural backgrounds

• Effective written and verbal communication skills, organization skills and time management

• Able to effectively organize and present information in group settings, both formal and informal

• Able to work effectively both independently (with minimal direct supervision) and collaboratively with various internal and external stakeholder groups

• Self-starter with a high level of initiative; flexible and adaptable

• Demonstrate sound work ethics and high standards of conduct and professionalism

• Possess a current, unrestricted Driver's License issued by the Commonwealth of Pennsylvania, valid insurance and have reliable transportation.

• Must be able to obtain Act 33/34 Clearances

Supervisory Relationships

This position functions as a home visitor and a member of the program team, with an advanced level of independence under the direction of a licensed social worker.

PHYSICAL DEMANDS

• The work performed is 80% school-based requiring sitting, keying, typing and answering phones, with occasional light lifting, carrying, stooping, standing, bending, walking and filing.

• Intense mental, visual and aural attention is required as the work involves responding to requests, planning or performing work that is at times, can be fast-paced.

• The other 20% of the position is conducted in the field. Candidate must possess the ability to work at locales outside of the office environment.

• Out of town and overnight travel may be required on occasion.
