Description of Award
The South - West Kiwanis of Peoria Leadership & Community Service Award is available for all 4-H members. The purpose of the award is to recognize and reward youth for exhibiting outstanding leadership & community service participation during their 4-H involvement.
Eligibility
Applicants must be current Peoria County 4-H members age 8 - 18.
Award Recognition
Two award recipients will be chosen each year. One recipient will be selected from the 8 – 12 year old age group and another will be selected from the 13 – 18 year old age group. Each recipient will be presented with a $250Award. The award will be presented during the Peoria County 4-H Recognition of Excellence Awards Program held in November each year. Recipients will also be invited to attend an Award Day at a South-West meeting of members.

The South – West Kiwanis Club of Peoria is the sponsor of the 4-H Leadership and Community Service Award. The award was created to recognize and honor youth for their commitment of service to others through leadership and community service projects.

Scholarship Application Form
Applicant Criteria:
1. This scholarship is available to all current 4-H members age 8 – 18.
2. There will be two awards presented. Awards will be for $250 each and paid directly to the award recipient. One award recipient will be selected from the 8 – 12 year old age group and the 2nd award recipient will be selected from the 13 – 18 year old age group.
3. Applications must be complete and include the following:
• Completed front page of personal information (name, address, etc.)
• Maximum of four (4) pages (does not include the front page), single sided that provide answers to questions included in the application.
• Two (2) written recommendation letters, enclosed in sealed envelopes with the applicant’s name on the front of the envelope. One recommendation must be from a 4-H volunteer or a U of I Extension staff member; the second recommendation must be from school staff member or an individual in your community. Recommendation letters should not be from individuals related to the applicant. Recommendation letters should focus on leadership demonstrated and community service involvement of the applicant.
➢ Include complete contact information on individuals who wrote recommendation letters (name, address, phone # & e-mail).
Completed applications & recommendation letters must be received no later than September 30 of the year applying. The winner will be notified by October 30 with details of the recognition. The award will be presented by a representative of the S-W Kiwanis Club during the Peoria County 4-HRecognition of Excellence Awards Program in November.

Award Application Questions
Applicants must answer the following questions in order to be considered for this scholarship. You are limited to four (4) single sided pages (does not include the front page with personal information). If typed, it should be no smaller than 10 point Times New Roman. Margins must be a minimum of ¾ “on all four sides. Questions and pages should be numbered.
1. List your major leadership roles in 4-H, School, and/or other (could include church leadership roles too). Leadership could include but is not limited to, committees and offices held. Also, include a brief description of your responsibilities and the year performed with each leadership role you list. For 4-H Leadership roles please provide the level of participation behind each role (L= Local Club: C = County: M = Multi-county: R = Regional: S = State: N = National: I = International) (20 points)
2. List the community service projects you have participated in with 4-H, school and/or other groups. Include a brief description of the project and the size and scope of the project impact on the community if possible. Also, include the year performed with each community service project you list. For 4-H community service projects please provide the level of participation behind each project (L= Local Club: C = County: M = Multi-county: R = Regional: S = State: N = National: I = International) (20 points)
3. Summarize your most important 4-H accomplishments. (10 points)
4. Highlight your school, church and/or community activities. (10 points)
5. Tell us about you. What else should we know about you when considering your award application? (15 points)
6. What are your future educational and career plans? (5 points)

Tips for Preparing Your 4-H Award Application
Specifications:
• Printed or done on computer
• 81/2 x 11 inch white paper
• Font no less than 10 but preferably 12 characters per inch (Times New Roman)
• Margins – 1 inch minimum on all 4 sides
• Maximum of 4 pages (one side only) The 4 pages does not include the front page with personal information.

What is Leadership? (20 points) [pic]
• Leadership means:
✓ Giving direction
✓ Planning
✓ Teaching
✓ Organizing
• Acting in a supporting role such as helping, assisting does not score as highly.
What is Community Service? (20 points) [pic]
• Assisting with the improvement of something in the community
• Can include individual or group projects that benefited the community
• Definition of impact: Measure of the tangible and intangible effects (consequences) of one things or entity's action or influence upon another. In other words – how will the community service project affect the lives of those in your community?

Summary of 4-H accomplishments (10 points):
• 4-H Accomplishments in project areas (should include size, growth of project/ knowledge & skills gained).

• Growth shows a progression from the first year to the present:

Future Education & Career Plans (5 points)
• As of right now what are your sort term and long term plans or goals for your future?
Recommendation Letters (15 points)
• Ask someone who knows you and is familiar with what you are involved in but not a relative
• When asking someone to write a letter for you, be clear and concise on what should be included in the letter & when the deadline is to submit the application and letters. (Don’t wait till the last minute to ask!)

• These points may be given at the judging committees discretion
