The CBHW addresses goals for Medicaid recipients specifically in the following areas: independent living, learning, working, socializing and recreation. The CBHW must be available to work Monday – Friday during normal business hours. A flexible schedule may be occasionally necessary for extended hours in the evening or on weekends. The CBHW may provide services in the agency's office, outside at community locations, and/or in the homes of clients.The CBHW will promote the development of social and daily living skills, education in co-occurring illness (SUD/MH), linkages to natural supports; assist in teaching symptom monitoring and illness/disorder self-management; and provide support in crisis situations.The Community Behavioral Health Worker (CBHW) is a paraprofessional class and is distinguished from licensed and case management personnel in that the Community Behavioral Health Worker does not assume primary case responsibility for Mental Health or Substance Abuse clients; other staff will be responsible for functional assessments and preparing individualized service plans (ISP). The CBHW may work with clients having substance use disorders, mental health disorders, or co-occurring disorders. The CBHW may be assigned to a team or work unit (e.g., Amador Recovery Project) or to certain therapist(s). Reports to the CCSS Program Director.Major Activities and End Results:Application assistance with Medicaid, SSI/SSDI, permanent supportive housing, Dona Ana County indigent programMonitor housing laws, regulations and policies for assigned clients receiving housing financial assistance: landlord/tenant relationships, tenants’ rights, advocacy for consumers and strategies for accessing and maintaining housing; perform regular field visits for house checks and monitoring level of client functioning as assignedRegister new patients for the AHC primary care clinic servicesEnroll new clients in the ARP program, conduct orientation with client handbookAdminister SUD/MH screening/assessment tools or surveys as trainedFacilitate support groups or classes as trainedOffer enabling services to improve clients’ social determinants of health (i.e., socioeconomic status, access to health care, and the physical environment or neighborhood where one resides); enabling services may include case management, benefit counseling or eligibility assistance, health education and anticipatory guidance for lifestyle issues, risk assessment/reduction and referrals, interpretation/translation, health literacy, outreach, transportation, patient medication assistance/adherence and education of patients and the community regarding the availability and appropriate use of health services.Pick up medications at a retail pharmacy and deliver them to patients; may also perform directly observed therapy (supervised dosing) in the community setting for priority patients, particularly ones with co-occurring disorders (SUD/MH).Accompany clients to medical and human/social service appointments.Participate in multidisciplinary team meetings and advise staff of changes in clients’ health status and/or recovery progressRefer clients to local health/social resources as needed and verify dispositions of referralsAnswer telephone and text inquiries to program helpline; assist with front desk reception tasks when neededProvide harm reduction services with syringe disposal/exchange, Narcan education/distribution, and HIV/HCV rapid testing in the office and in field/community.Maintain timely and accurate Progress Notes in an electronic health record (EHR)Experience, Knowledge and SkillsSkill for continuous self-study learning/education. Skill with combining lived experience and learned experience to support clients. Skill with maintaining professional boundaries with clients. Knowledge of the importance of fidelity with provision of evidence-based practices for recovery (e.g., motivational interviewing, harm reduction, SBIRT, medication assisted treatment (MAT), counseling/CBT, trauma informed care). Ability to be active, integrated team member. Ability to promote a culture in which each consumer’s point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation, and community self-help activities. Ability to work with and provide services to a wide range of culturally and linguistically diverse persons.Ability to maintain client confidentiality according to HIPAA and 42 CFR Part 2.Be proficient in Microsoft Word and Excel.Qualifications:Minimum: Must be at least 18 years old. Hold an associate’s degree in human/social services and have a minimum of two (2) years of experience working with the target population (i.e., homeless or persons who use drugs) OR Be certified as a Peer or Family Support Worker from the NM Credentialing Board for Behavioral Health Professionals with one (1) year of experience working with the target population (Preferred). Bilingual (English/Spanish) preferred.Must be willing to use personal reliable vehicle with mileage reimbursement for fieldwork in the course of employment. Must possess and maintain a current valid Driver's License in the state where candidate is residing. Must maintain a clean driving record. Must provide current personal owner liability automobile insurance and must maintain the insurance coverage current throughout the course of employment in this position.Note: Within 90 days of employment the Community Behavioral Health Worker (CBHW) must successfully complete the 20-hour Comprehensive Community Support Services (CCSS) course from a state designated training unit. Must obtain an NPI number and NM Medicaid provider number as non-independent Behavioral Health Worker.
