INSTRUCTIONS FOR COMPLETING THE USAID CONTRACTOR EMPLOYEE

BIOGRAPHICAL DATA SHEET (EBD)

The USAID Contractor Employee Biographical Data Sheet is required for all candidates proposed for work on USAID contracts. The form is a legal document that is used by USAID to evaluate the candidate's proposed salary. DAI is required by USAID to verify the accuracy of the information contained in this form. There may be civil and/or criminal penalties associated with submitting a form that contains false information, or that has been inadequately verified. DAI will propose a salary which it believes is justified by the candidate's education, employment and salary history; however, final approval for all proposed salaries is given by USAID.

Please complete the form with the following information (and see sample EBD):

1. Name (last, first, middle initial): completed by candidate.

2. Contractor's Name: completed by DAI.

3. Employee's Address: completed by candidate, and should be the candidate's home address, not place of employment.

4. Contract Number: completed by DAI.

5. Position under Contract: completed by DAI.

6. Proposed Salary: completed by DAI.

7. Duration of Assignment: completed by DAI.

8. Telephone Number: completed by candidate. For candidates living outside of the U.S., a country and city code should be included.

9. Place of Birth: completed by candidate, please include city and country of birth.

10. Citizenship: completed by candidate. Non-U.S. citizens living in the U.S. should give visa status.

11. Names, Ages, and Relationship of Dependents to Accompany Individual to Country of Assignment: completed by candidate; include legal dependents only.

12. Education: completed by candidate.

13. Language Proficiency: completed by candidate, using FSI rating scale. 2=limited working proficiency, 3=general proficiency, 4=advanced professional proficiency, and 5=functional native proficiency.

14. Employment History: completed by the candidate. Must include employment and salary history for the last three years. For each employer listed, you must include the name of the organization, a point of contact, and contact number. The salary must be reported on an annual basis, and must be listed as the base salary, not including bonuses, profit-sharing, commissions, overtime payments, overseas differential, housing, cost of living or dependent education allowances. The salary should be reported in the currency in which it was received

It is important to report the each salary change within a job or company. Please see sample work history below. If an individual has worked for the same employer since 2003, and has received annual salary increases, the form should be completed like the sample form.

15. Specific Consultant Services: completed by candidate. This includes consulting assignments for the last three years. Requires brief description of services performed, employer's name, point of contact, and telephone number, dates of employment, number of actual work days, and daily rate. The daily rate reported should be the base rate paid, exclusive of any per diem. The daily rate should be reported in the currency in which it was earned.

16. Certification: you will be required to sign the form once DAI is actively considering you for a specific position.

17. Contractor's Certification: to be signed and dated by the Contracting Staff at DAI, after the DAI staff verifies the information contained in the form. Note that in signing this form, DAI certifies that it has taken reasonable steps in verifying the information. Certifications that are false, or that are based on inadequately verified information may result in remedial action by USAID, ranging from refund claims to criminal prosecution.
