SUPPLIER REGISTRATION QUESTIONNAIREOnce a Supplier has received an invitation from SAP Ariba to proceed with the registration process, they need to complete the Supplier Registration Questionnaire online.The Supplier Registration Questionnaire contains 16 sections, each comprising questions that require specific answers and/or supporting documentation that must be uploaded to SAP Ariba.Section 1: Infractions that lead to disqualificationThe Supplier must download the infractions document from SAP Ariba detailing infractions that may lead to disqualification. Once the Supplier has read the content, they must confirm understanding of the content. The Supplier’s Company site will undergo a physical assessment based on the Commodity selected and be informed of the physical assessment date. The Company understands the contents and provisions of this undertaking and that any intentional or negligent disclosure of false information by myself/ourselves and/or my/our consultants, partners, members, associates, employees, contractors, sub-contractors, agents or sub-agents, whether permanent or temporary, shall cause irreparable harm and give rise to appropriate recourse by Debswana against the offending party. ................
