2015 OPEN ENROLLMENT FOR HEALTH & DENTAL INSURANCE Health, Dental, Flexible Spending Accounts Coverage effective dates: 8/1/2015 to 12/31/2015 Flexible Spending Acct effective date is 7/1/2015

Highlights include:

It is Annual Enrollment time again!

Medical Insurance rates will not change for Cigna covered employees and covered spouses who completed the biometric screening.

There are no major changes in your medical plans. A comparison chart is posted to the website and will be at the meetings.

Voluntary Delta Dental and Dominion Dental will continue at the same rates. RPS has a new benefits broker, Mark III Employee Benefits. Mark III is an Employee Benefits firm that has

worked in the public sector since 1977. They currently serve many public sector entities in Virginia and across the southeast. Since this is the first year these new products are being offered, coverage will be "Guaranteed Issue" (No Health Questions) during this year's enrollment. In addition, when you are transitioning from a current policy, there will be no pre-existing condition clause, if you enroll this year! Mark III is planning to be onsite at every RPS school to meet you. They are working with your principal/administrator to schedule these meetings. This will be a short plan year effective 8/1/15- 12/31/15. The health insurance plan year is changing to January 1 effective 1/1/2016. We will be conducting another enrollment in the Fall to accommodate this change. You will have an opportunity to change your coverages again for the start of this plan year.

Beginning Tuesday, May 26th through Friday, June 19th, all benefit-eligible employees will be asked to : Attend a group meeting with an overview of the benefits being offered for the plan year Meet individually with a Mark III Benefits Consultant (you will be able to schedule this meeting at the group

meeting) Review current benefit elections Make necessary benefit changes

Deductions will be discontinued effective June 30th for the following plans: Policies offered by Colonial, Colonial Life, Transamerica, Kanawa/.Conseco Plans, Protective Life, and Washington National.

A schedule with specific meeting times and locations will be posted to the benefits website and at your work location. This will be the only time during the year that you can make changes in your benefits unless you experience a qualifying event outlined by the IRS (marriage, divorce, birth of a child, etc.).

A Brief Benefit Description of new plans being offered by Mark III

Humana - Cancer Humana will be the new carrier for the Supplemental Cancer coverage. This cancer policy provides cash benefits paid directly to the insured for cancer treatment based on a schedule of benefits listed in the policy. During this enrollment, the coverage will be guaranteed issue on yourself as well as your dependents. If you or your dependents have had a previous challenge with getting cancer coverage, now is your opportunity to apply without answering health questions.

AFLAC Group - Accident Similar to the cancer policy, the accident policy pays cash benefits directly to the insured, based on a schedule of benefits listed in the policy, for medical treatment due to an accident. You may cover yourself as well as your dependents.

AFLAC Group - Critical Illness The Critical Illness plan provides a lump sum cash benefit directly to the insured for the diagnosis of a covered illness such as heart attack, stroke or kidney failure, to name a few. You can receive a check for the benefit amount that you selected when you applied for the plan. This benefit also includes a Health Screening Benefit which allows you to collect $100 each calendar year if you have specified health screening tests performed. The Health Screening Benefit will also apply to your spouse, if covered on the plan. Coverage is available for you and your dependents.

AFLAC ? Hospital Indemnity (Gap Plan) Provides 24 hour protection Issue age is 18 ? 64 Spouse can be covered from age 18 to 64 Pays cash benefit for accidents or sickness. Pays for hospital confinement, surgery and anesthesia benefits, and a $50 wellness benefit. 2 Plan Options available Low & High

Texas Life - Whole Life Solutions 121 whole life guarantees: Paid up insurance at age 65 or after 20 years, if you purchase the policy at age 46 or later. Level Premium Death benefit Permanent and portable when an employee changes jobs or retires. Solutions 121 offers GI amounts to employees by just answering one question- "Are you actively at work?" See Tier 1 for GI amounts. Tier 2 asks yes/no questions so employees will know on the spot if they are approved or not. Employees do not have to take out insurance in order to cover dependents. All Solutions 121 plans for Mark III include: Accelerated Death Benefit due to Terminal Illness Accelerated Death Benefit for Chronic Illness Waiver of Premium Rider (for ages 17-59)
