Inclusion Criteria for IV to PO Conversion:
Infections that Require IV Antibiotics
Must satisfy below criteria:
? CNS infections (e.g. meningitis brain/spinal abscess)
? Neutropenic fever
? Tolerate oral diet or enteral nutrition
? Orbital cellulitis
? Pancreatic necrosis or abscess
and/or receiving oral medications
? Endocarditis
? Bloodstream infection due to Staphylococcus aureus,
? Infection does not require IV antibiotics
? Mediastinitis
Pseudomonas, Enterococcus, or Candida
? Afebrile (< 100.4?F in the last 24 hours)
? Osteomyelitis
? Endophthalmitis
? Received 24 hours of IV antibiotics
? Gangrene
? Cystic fibrosis exacerbation
? Documentation of clinical improvement
? Empyema
? Non-neutropenic (ANC > 500)
