 -104774114300School Screening Basic Protocols* Comment on Medical Form & Complete ‘Hoja de Referencia’ for all Referrals Weights: Make sure child has shoes and jacket off when being weighed. Record weights in kilograms to the nearest 10th. If in pounds, convert to kilos [Pounds / 2.2 (kg)]Heights:Make sure child is measured with shoes off. Record height in cm’s [1 inch = 2.5 cm.]Have child looking straight ahead, head not tilted. Use tongue depressor or ruler to mark.* If you are concerned about a child’s weight or height, consult the appropriate WHO Growth Chart. If child is above the 95th percentile or below the 5th percentile, make a comment on the medical form & flag for follow-up.Skin Check:Visually inspect skin including face, arms, legs, and trunk when possible. Look for signs of broken skin, rashes, infections.DO NOT REFER for insect (mosquito) bites and dry skin unless skin is compromised or bite reaction indicates possible secondary infection. Comment on medical form & refer for s/s of skin rash, infections, and scabies. Heart:Listen to and record pulse/heart rate per minute on medical form.Auscultate & note abnormal or irregular heart sounds. Consult with MD and refer as needed. Lungs:Listen to and record number of respirations in one minute on medical form.Auscultate lungs & note abnormal lung sounds. If abnormalities persist after coughing & listening a second time, please consult with MD and refer for follow-up if necessary.Albendazole:Albendazole, 400 mg PO every 6 months for ascaris/worms. (If UTD, Vitamin is given instead.)Dental Evaluation:Visually inspect all upper & lower teeth.Record any abscesses or broken teeth on medical form, HIGHLIGHT, and refer for follow-up.Record any pitting and cavities/caries on medical form and refer for follow-up.Educate children on proper brushing techniques: Short strokes, move back & forth against the teeth & gums, around the surface of each tooth. Use the tip of the brush to reach behind the back teeth. Brush for at least 2 minutes :)Fluoride Varnish:Ensure child brushes teeth prior to applying varnish.Use one package of fluoride varnish & 1 applicator per child.Paint varnish on molars & teeth, beginning with bottom molars. Varnish hardens upon contact with saliva. Inform children it is normal to feel the protective varnish on their teeth.Avoid eating for at least 1 hour after fluoride application. ................
