COMMUNITY DEVELOPMENT CENTERJOB DESCRIPTION (July 2018)FLSA: ExemptJob Classification: 6JOB TITLE: Independent Support Coordinator/ECF CHOICES Counselor SupervisorREPORTS TO: ISC/ECF Program Manager DEPARTMENT: Independent Support Coordination/Employment and Community First CHOICESJOB PURPOSE:To provide supervision, support and guidance to the Independent Support Coordinator/ ECF Counselor staff and Administrative Assistant with daily responsibilities. To ensure the provision of necessary and effective services to the individuals contracted by the agency. To assist the Program Manager with program compliance and standard operating procedures. QUALIFICATIONS:The Independent Support Coordinator Supervisor must have a four-year degree in order to supervise any ISC staff without a bachelor’s level degree (DIDD-Operations Manual Chapter 4, Rev. 2014) The ISC supervisor should have a background that may include, but is not limited to, the following:Prior supervisory/management experienceKnowledge of DIDD Policy & ProcedureWork experience as an ISC or similar position providing services to ID/DD IndividualsStrong organizational and problem-solving skillsSolid written and verbal communication skillsComputer knowledge, specifically word processing, Excel and the InternetESSENTIAL JOB FUNCTIONS:Supervises and monitors the activities of the Independent Support Coordinator/ Employment Community First Counselor staff, which includes, but is not limited to the following:Weekly Scheduling & Planning to meet routine deadlinesISP: developing annual ISP calendars, development/ completion/ execution and monitoring of the planning meeting document. Development of Service Plans and follow upCompletion of Support Notes per DIDD and CDC policies for contact and documentation of FF, HV, and Reviews Caseload assignments and distribution Work at Home submissions & schedulingCDC QAM submissions, Internal Audit follow up Supervises and monitors the activities of the Administrative Assistant, which includes, but is not limited to the following:Completion and submission of the Agency Service Review (ASR) for revenue Billing.Timely acquisition and submission of Recertification documents“Current Standings” list for ISP pilation of critical data in a user-friendly, computer format file.QA methods and monitoringInternal auditsISP tracking and submission process per internal CDC policy Coordinates on-call schedule, procedures and follow-upIncident Management procedures and follow upAssists and supports the Program Manager with program responsibilities, which includes, but is not limited to the following:Review of annual planning (ISP) documents completed by ISC staff before those plans are sent to the DIDD Regional Office for review and approval. Completing reports as requested by DIDD, TennCare, CMS Center for Medicaid and Medicare Services or any other offices of authority.Reviewing and analyzing manuals, guidelines and reports to present critical issues to staff.Participating in meetings, conferences or training as piling data for budget concerns.Provides information regarding services, programs, policies, plans and other pertinent data to individuals, families, conservators, advocates, providers and members of the community.Conducts regular staff meetings with a formal agenda.Oversees direct personnel issues related to staff in the ISC office location of responsibility including staff evaluations, performance reviews, warnings, and recognitions. Assumes caseload responsibilities during an ISC vacancy such as separation from the agency or an extended absence from the agency.Assumes essential office responsibilities during a vacancy of an office support person such as separation from the agency or an extended absence from the agency.Performs the steps necessary in filling a vacant position, which includes, but is not limited to the following:Recruiting potential candidatesReviewing resumes and applicationsInterviewing applicantsHiring an individualTraining the new hire in Support Coordination responsibilities Coordinating training for additional requirements (DIDD and new hire orientation) Conducting evaluations throughout the probationary periodProvide direct guidance through probationary period of initial ISC staff hiring INTENSIVE SUPPORT COORDINATION OVERSIGHTThe Supervisor meets with the ISC monthly to review the Supervisor’s Monthly checklist identifying any issues of intensive supports needed, support coordination needs of all the individuals served by the ISC of CDC providing guidance and direction to ISC staff. The Supervisor monitors the intensive support coordination services provided by the ISC of CDC to consumers on each caseload. ESSENTIAL JOB FUNCTIONS AS THE ISC: The Independent Support Coordinator provides information regarding services, programs, policies, plans and other pertinent data to individuals, families, conservators, advocates, providers and members of the community.The Independent Support Coordinator advocates for the individuals and families.The Independent Support Coordinator coordinates services and supports, which includes, but is not limited to the following:Ensuring that needed assessments are obtainedTracking the scheduling and completion of needed evaluationsLocating and accessing needed supports and servicesDeveloping community and natural supportsAssisting with programmatic and fiscal eligibilityThe Independent Support Coordinator monitors services and supports, which includes, but is not limited to the following:Service Providers are in compliance with Federal & State regulationsThe individual exercises his/her rights and rights are not violatedThe individual lives and works in an environment of his/her preferenceThe individual’s environment is maintained in a quality mannerThe individual’s finances are managed in a fair and equitable mannerThe Independent Support Coordinator documents support coordination services, which include, but are not limited to the following:Composing the Individual Support Plan (ISP):Information gatheringDevelopment (service plans)CompletionDistributionCompleting annual and ancillary Service PlansCompiling support coordination activities in monthly Support NotesConducting quarterly Home Visits with individuals and/or familyConducting monthly Face-to-Face visits with the individualMonitoring ISP outcomes for execution and satisfactionMaintaining hardcopy case records and computer file recordsThe Independent Support Coordinator facilitates annual pre planning and ISP meetings as well as other meetings as necessary.Performs On-Call responsibilities and follow-upPerforms other responsibilities as necessary.REQUIREMENTSKnowledge: Requires an understanding of human behavior, parent centered approaches, problem solving and helping skills, community resources, communication skills both written and verbal, as well as supervision skills usually attained through completion of at least a Bachelor’s degree and 4 years related experience working with individuals, families, and adults.Mental:Requires normal attention with periods of high concentration intermittently to completing case reports and support note plans approximately 25% of time. Requires capability of dealing with periods of interactions with clients in stressful situations approximately 25% of time. Requires normal attention and interactions with clients served in teaching, education, and case management of individuals, adults, and families approximately 25% of time. Physical: Requires walking/standing approximately 25% of time, standing approximately 25% of time, driving an automobile approximately 25% of time, lifting packages weighing up to 50 lbs. Approximately 25% of time. Environmental: Requires capability of performing essential job functions in established officeenvironment under normal lighting and climate control tolerances. Also requires ability to maneuver in environments, which are not handicapped accessible such as private homes and apartments.
