PLEASE CONTACT THE STATE BOARD OFFICE IN THE STATE THAT YOU WERE ORIGINALLY LICENSED AND HAVE THEM MAIL THIS INFORMATION DIRECTLY TO OUR OFFICE.
4. TOTAL FEE OF $50.00 FOR A LOUISIANA RESIDENT - $75.00 FOR A NONRESIDENT APPLICANT
5. A COPY OF YOUR DRIVER LICENSE OR IDENTIFICATION CARD ALONG WITH A COPY OF THE SOCIAL SECURITY CARD.
6. ALL APPLICANTS MUST TAKE A STATE WRITTEN EXAM.
