What You Will Need: Word Processing Application, Like Word, Open Office, or Google Drive Adobe Acrobat 1. Creating a scratch form It is simple enough to create a PDF form yourself. The advantage of doing everything alone is that you get to check exactly how everything in your shape will look. While, with a model, you are tormented by what is already there. If you create your form yourself, you can do it in whatever you like. You will see in the screenshots that I am doing a very basic module for the purpose of this tutorial, but you can get as creative as you like. Step 1: Create a Document Create a module bone in the word processing of your choice. You can use anything you canuntil you can export the finished product as PDF. Make sure that when you are spreading the document, you leave room for the form fields. Everything you need to worry about right now is the labels on the field. I created my document in Google Drive, but you can use and word processing application. Export or save as PDF. Export your document as a PDF and save it where you can find it. Step 2: Create a new form in Acrobat Open Acrobat and select the Create Form task. When asked to choose the option to create the form from the existing document and import the PDF we created in Step 1. Click Create Form in Select an Task. You can also reach the field properties by right-clicking the field in the document and selecting Properties. There are different preferences to change for each type of field. In the text field, set a default value for the field in the Options from tab to give users short instructions. The tab Appearance allows you to change the character and size of the text for the field; this is important if space is limited so that the entry text does not impact at the end of your field. Go to Options via Field Properties. The Options tab in Properties for both control boxes and radio buttons allows you to choose whether a box or button is selected by default. The appearance will change as the bullet looks, if the edge is thick or thin and if there is a fill color. For the list boxes and the surveys the Appearance tab does practically the same thing here as it did for text fields; you can change the font, text size, or even regularOptions are what is really important here, though. You will only have some blank boxes with nothing to select if you do not add list items in the Options tab. Step 5: Save Give a last look around. If everything seems good, save the file as a PDF. You can verify that everything worked well by opening your finished PDF preview or any other PDF viewer that will not begin to try to change the fields of the module.
