1770380-68072000St Peter & St James Hospice Wellbeing Services – Referral CriteriaReferrals to Beacon View Wellbeing Centre can be made by any healthcare professional. Please see below for our eligibility criteria: The patient is aged 18 years or overThe patient has given specific consent for the referral The patient lives within the hospice catchment areaThe patient has a life-limiting condition, with a prognosis of up to two yearsThe focus of the patient’s treatment is palliative, with the aim of improving quality of lifeThe patient does not have a significant cognitive impairment and is able to communicate their needs and wishes effectivelyThe patient’s level of mobility is such that they do not require a full hoist on transferThe patient would benefit from groups and activities to support their psycho-social needs.Whilst we understand that a secondary benefit of referral may be respite for their carer(s), this must not be the primary reason for the referral.If the patient meets all the above criteria, please download, complete and submit the referral form (please see contact details below).If the patient does not meet all of the eligibility criteria but you feel that they should still be considered for referral, please contact us to discuss any extenuating circumstances. What happens next? Once the referral has been approved, patients will be contacted to arrange an individual assessment. If we can support the patient effectively, a ten week programme will be tailored to their needs.
