Policy Statement: Evidence based clinical careDescription: PICU Department Clinician performed Point of Care Ultrasound (PoCUS) diagnostic scansIndications: VariousContraindications: NilWho must comply with this procedure?MH PICU CliniciansMH PICU PoCUS faculty MH Sonographer Educators PoCUS program – Monash Imaging & MonashHeartThis procedure applies in the following setting This is applicable to all MH patients in PICU deemed appropriate for PoCUS performed by PICU cliniciansPrecautions and ComplicationsMH Infection Control policies & procedures applyMH OH&S policies & procedures applyEquipment PoCUS ultrasound machines formally installed into PICU, connected to PACS image management systems (Monash Imaging Carestream & MonashHeart Intellispace)Ultrasound equipment serviced & maintained by PICU, support available from Monash Imaging & MonashHeart as requiredProceduresScope of practise for PICU Clinician performed PoCUS is currently limited to the following examinations:BLADDER (Volume)^LUNG (Pleural effusion)^FOCUS (Effusion, Ventricular size, Function assessment)*^Training & credentialing by Monash Imaging* Training & credentialing by MonashHeartNote: Ultrasound guided procedures are outside the remit of this document and are the responsibility of the PICU departmentFurther scope of practise development may be negotiated by PoCUS faculty, in consultation with Directors of PICU, Monash Imaging & MonashHeartList of Implementation Tools No clinical management decisions or interventional procedures should be based on PoCUS findings without engagement of the PICU ConsultantNo diagnostic ultrasound examinations shall be performed beyond agreed scope of clinical practise [see PICU Bladder US Accreditation 2019, PICU Chest US Accreditation 2019 & PICU FoCUS Accreditation 2019 documents]Management decisions based upon PoCUS imaging must be reviewed by a PoCUS accredited consultant and/or formal imaging confirmationThe performing clinician shall save images of the examination per scan protocolThe performing clinician shall complete a worksheet of ultrasound scan findings for every examination, scan/email to PoCUS program & file original worksheet in patient notesAll PICU diagnostic ultrasound scans will be uploaded to Monash Imaging or MonashHeart PACS systems for quality audit & clinician electronic logbook maintenance purposes The PoCUS governance group (composed of Sonographer educators, PICU Consultants, PICU medical educators, PICU Director) will meet regularly to provide internal governance for the program; engaging in matters including credentialing approval, education planning, clinical quality case review, peer to peer support, program compliance, development of new scopes of practise & equipment issues Additional ResourcesMH Training & Accreditation documents for BLADDER, CHEST and ECHO modules References:AUSTRALASIAN SOCIETY FOR ULTRASOUND IN MEDICINE (2008) Policy B8 Statement on the Use of Ultrasound by Medical Practitioners.
