GPRA Client ID Convention: The first six digits of the GPRA IDs are the site IDs provided in the table to the below. Client IDs can be up to 15 alphanumeric characters - do not include hyphens when creating GPRA Client IDs. The remaining digits are site-assigned (you could use your EHR client number or internal client indentifier). If your site does not have a site ID listed below, please contact The Evaluation Center.
