CCI has been a provider in the Mid-Shore region for over 35 years

Residential Rehabilitation Program Psychiatric Rehabilitation Program Supported Employment Services Healthy Transitions- assisting those in the TAY age range with making an

effective transition through care coordination services

Day Programs conveniently located near our Residential Program Sites in Dorchester, Kent and Queen Anne's Counties.

Corsica River Mental Health Services- our Clinical Outpatient partner provides caring, effective, and highly accessible assessment and treatment to all mid-shore residents. They have clinics in Centreville, St. Michaels, Federalsburg and Cambridge, Maryland.

Intensive- Overnight Intensive General Medically Fragile/Aging in Place Program

Health Home Program

Offers individually focused care, empowering participants to manage and prevent chronic conditions, with a goal of improving overall health and reducing avoidable hospital visits.

Supported Employment

helping individuals participate in competitive work in integrated work settings that are consistent with the strengths, resources, priorities, concerns, abilities, capabilities, interests, and informed choice of the individuals.

Expectations

This is a PROGRAM, not just housing All clients are expected to be in an approved structured daytime

activity (employment, recovery center, volunteer) Follow all responsibilities and expectations of RRP

Including, smoking in designated areas

All clients are expected to pay Cost of Care, CCI utilizes the approved Maryland formulation for determining COC.

Participate in Mental Health clinical treatment, permitting collaboration between OPMHC and CCI.

If applicable, Fulfill responsibilities for Parole and Probation and/or Conditions of Release
