Crossroads' Program Highlights & Accomplishments

Substance Abuse services received a grant from the CENTRA Foundation to provide a SA Peer Recovery Specialist, open access substance abuse evaluations and transportation to Recovery groups at the Farmville clinic.

Crossroads and Town House, agency's psychosocial rehabilitation program, sponsored the 3rd annual Recovery Walk on the High Bridge Trail (state park on old rail bed). Four other CSB programs participated.

Relapse Prevention groups held at Town House Psychosocial Rehabilitation Program. Agency hired a child/adolescent psychiatrist to provide on-site medication management services in the

Amelia Clinic and tele-med services in Charlotte, Lunenburg and Prince Edward clinics. Supported Employment program received a 3 year CARF accreditation. Agency went through a licensure review and is scheduled for a triennial license through 2017. Agency provided behavioral health services to area Head Start programs. Hired two substance abuse case managers to expand community outreach efforts. Expanded substance abuse treatment programs to address men with trauma who abuse substances

and another group to address people with depression/anxiety who abuse substances. Continued development and addition of programs to the Credible Electronic Health Records. ID

residential and day support services were included. Clinicians completed 1200 assessments/evaluations for individuals requesting mental health or

substance use outpatient services.

Clinical Services applied for and was awarded a One Time Criminal Justice Diversion funding grant to provide free CIT training and Mental Health First Aid training to law enforcement officers.

Two clinical staff members received Mental Health First Aid Adult Instructor Training in March 2014 and were certified to provide Mental Health First Aid training to community agencies/members.

Emergency Services and other clinical staff members completed 832 Crisis Intervention/Pre-Screenings for inpatient psychiatric hospitalizations and 112 re-commitment pre-screenings and hearings for

Piedmont Geriatric Hospital. Town House

o Member attendance increased by 17% from 7/13 to 6/14. o Passed licensure audit. To quote the licensing specialist ? "the program is a well-oiled

machine". o 4 student interns from Longwood University's Schools of Social Work and Psychology. o Article published on website on the Town House Hiking Club. o Participated in the Coalition Rally at the State Capital.

Recovery o Held the 3rd Annual Recovery Walk on High Bridge Trail ? with walkers from Hanover, Henrico, Chesterfield, Richmond and Goochland-Powhatan joining us. o Published articles on mental health recovery in local newspapers. o Agency's Recovery Committee organized training for case managers to implement Advance Directives for individuals receiving services.

Began offering a Crossroads Speakers Bureau to groups and organizations in the communities we serve. Crossroads is a member of the Piedmont Health Coalition and is represented on its Provisional

Leadership Team.

Crossroads' Program Highlights & Accomplishments

Success with all of the individuals received from the DOJ settlement can be considered an accomplishment for all programs. Maintaining all of them in the community for the entire year with all of the medical and behavioral complications is the best accomplishment for Intellectual Disability Services.

Individuals have a positive impact and are truly part of the community in many ways such as being

recognized as the "Faces Aces" with a banquet and gifts honoring the work our individuals do for the local food bank. Our individuals are members at the Farmville YMCA and exercise alongside community members. All programs had individuals participating in the Christmas Parade ? individuals and staff walked and rode floats, being part of the community. 9th year of running Camp Unity ? more participation than ever and snagged 5 great staff from the hiring; had artwork displayed at Longwood University's Visual Arts Center. Coached Special Olympics champs this year winning gold, silver and bronze in basketball at State Championship 6 Respite admissions, 2 moving into permanent placements. All respites successful and led to a decision to open Maryland House as a Respite Home. Supported Employment continues to face fiscal challenges due to the costs of doing business WHILE providing solid training / supervision for the individuals involved. Longwood University and the YMCA are among our steady customers. Our relationship with the area schools continues to solidify and several students have been evaluated and are now working at Supported Employment.
