Spring 2021 Generic Deadline DatesAll requirement deadlines must be met in order to be eligible to register for NURS 1871, 1140, and NURC 1104. Due by September 11, 2020Return by US mail – Acknowledgement Form and Demographic Form.Must be received by: November 13, 2020 via US mail.Orientation Verification FormHIPAA Acknowledgement FormGraduation-Employment Relationship Acknowledgement FormNursing Student Handbook Acknowledgement FormCPR Card – Must be received by November 13, 2020.See specific instructions on CPR information sheet.Drug Testing and Background Check – Due by November 13, 2020.Follow the detailed information sheet. Print results from BCI and FBI must be returned and processed by CSCC by the above deadline. You must use this system. If you have previously tested for another CSCC program you must complete a new test/check. Background checks can take from 7 – 45 days. Do not delay starting this process. Friday, November 20, 2020 3 PM.You must have met all requirements for enrollment and be registered for required classes.Due by Monday, December 7, 2020 – Send to BookstoreSpecial order uniforms and name badge ordered.Uniforms must be purchased from CSCC Bookstore.Health Records: Due as directed by the Health Records Office.
