VENDORS,

The Elgin Math and Science Academy (“EMSA”) located at 1600 Dundee Avenue in Elgin, IL 60120 is soliciting vendors with an expertise in construction services-demolition to perform such services in EMSA’s Art Barn, an empty, currently unused building adjacent to EMSA’s main school building. This demolition project is considered Phase I of an ongoing renovation project for the Art Barn building.

A detailed description of this project’s requirements is itemized in the attached Request for Proposal (“RFP”). All bids must be submitted to EMSA’s CFO no later than 5:00 pm on Friday, November 2, 2018. Bids submitted after this time will not be considered.

The Art Barn is available to be seen by potential vendors by appointment only by contacting EMSA’s CFO whose information is listed below.

RENOVATION OF ART BARN PROJECT – PHASE I

I. Background

CONSTRUCTION DEMOLITION SERVICES

EMSA intends to use CONSTRUCTION DEMOLITION SERVICES to COMPLETE PHASE I OF ITS ART BARN RENOVATION PROJECT (“Project”). In general, the work shall include, but not necessarily be limited to, the removal of damaged surfaces (walls and ceiling); thoroughly clean the interior of the building; replace broken windows; replace the main entry door; and repair the roof, as may be determined.

Objectives for Demolition Work

The objectives to be achieved and other work-related requirements are more fully delineated in Section II, Scope of Work.

II. Scope of work

The Contractor’s Scope of Work for this Project includes the following work elements:

First Floor

• Removal of all interior ceiling sheathing and insulation

• Removal of suspended light fixtures

• Removal of all wallboard and sheathing

• Removal of sinks; cap back to source

• Removal of room dividers

• Removal of door unit

• Removal of front windows and associated construction

• Replace broken windows (number to be determined)

• Replace main door

Basement

• Removal of HVAC equipment and ductwork

• Removal of water heater

• Removal of toilet room partitions and fixtures; cap back to source

• Removal of interior wall sheathing

• Removal of ceiling panels and framework

• Salvage HVAC diffusers/grilles; electrical outlets; switches

• Clean out storage areas

• Replace broken windows (number to be determined)

Installation Work – General Instructions

All work shall be done at such times as EMSA shall deem appropriate. The day-to-day work schedule will be coordinated by the Office of the CFO at EMSA. Work shall not begin in any area without specific notification of, and approval by Frank Bisconti, CFO, or his designee.

III. program management

Direction

The Art Barn Renovation Project – Phase I shall be managed by the Office of the CFO at EMSA.

It is expected that informal daily/weekly progress and facilitation meetings will be held with the Contractor to keep the project timeline intact and to resolve any problems that might arise daily.

Schedule

EMSA intends to have work commence on Monday, October 22, 2018 and have this work completed as soon as professionally possible, no later than Friday, November 2, 2018.

V. Proposal EVALUATION criteria

EMSA will evaluate proposals and select a Contractor based on any combination or all the following factors:

• Qualifications and relevant experience of the firm’s proposed project management team

• Qualifications and relevant experience of the firm’s proposed staff

• The firm’s track record of successful completion of assignments like this Project

• Quality of references from similar recently completed work

• Understanding of the issues facing EMSA

• Quality of Contractor’s Proposed Work Plan

• The extent to which the Proposed Work Plan matches the needs of EMSA

• Quality of the Contractor’s knowledge work needed to be completed

• Reasonableness of cost of labor rates, products and associated services

VI. requirements and format of the proposal

Contractors responding to this RFP shall submit their proposals, organized and presented as outlined below.

Accuracy and completeness are essential. Since the successful proposal will be incorporated into the contract, Contractors are cautioned not to make claims or statements to which they are not prepared to commit contractually.

Part 1 – Letter of Transmittal

Part 1 of the Proposal must consist of a letter of transmittal signed by an individual authorized to bind the Contractor contractually. It shall:

• Concisely identify the services offered in your proposal

• State that the proposal will remain in effect for a period of thirty (30) calendar days after the deadline for submission of proposals

• Include the name, title, address, telephone number and email address of one or more project management contact individuals

• Include the name, title, address, telephone number and email address of one or more individuals who are authorized to sign a contract

Part 2 – Understanding of the Scope of Work

In this section, Contractor shall provide a concise presentation of its understanding of the major issues facing EMSA and of the elements required to implement the Scope of Work. Contractor will notify EMSA of potential difficulties that might arise during the implementation of the work and of major managerial or operational decisions that will have to be made during the term of the contract.

Part 3 – Proposed Work Plan and Schedule

In this section, the Contractor will describe the approach and tasks to be undertaken to successfully complete the requested work. Issues raised in Section VI, Part 2, shall be addressed, particularly concerning how such tasks will be managed and organized. A proposed schedule delineating the completion by task is to be included.

Part 4 – Estimated Cost to EMSA

In this section, Contractor will provide detailed cost estimates for labor, equipment, materials, hardware/software, and all other expenses related to the Scope of Work.

Labor hours should be identified by type of labor and hourly rates broken down by tasks described in the Work Plan. Other expenses are to be broken down in a similar fashion (when possible), by task as described in Work Plan.

Note: Please provide adequate details and/or supporting documentation as to how cost estimates and totals were calculated.

IMPORTANT INFORMATION REGARDING COMPLIANCE WITH THE ILLINOIS PREVAILING WAGE ACT: Any firm selected to render services under this contract shall pay the “prevailing wage” within the meaning of the Illinois Prevailing Wage Act, 820 ILCS 130/.01 et seq. (“Act”) that can be reviewed at .  The Act requires contractors and subcontractors to pay laborers, workers and mechanics performing services on public works projects no less than the current “prevailing rate of wages” (hourly cash wages plus amount for fringe benefits) in the county where the work is performed. The Illinois Department of Labor (“DOL”) publishes the prevailing wage rates on its website located at . The DOL revises the prevailing wage rates from time to time and the contractor/subcontractor has an obligation to check the DOL’s website for revisions to prevailing wage rates. For information regarding current prevailing wage rates, please refer to the Illinois Department of Labor’s website. All contractors and subcontractors rendering services under the awarded contract must comply with all requirements of the Act, including but not limited to, all wage requirements and notice and record keeping duties.

Part 5 – Proposed Project Team

In this section, staff members that will be assigned to Project should be identified, as well as their title, labor category and roles in carrying out the Proposed Work Plan. This section should highlight why Contractor feels that the selected team members are ideally suited for this Project. Team member biographies (detailing relevant experience and qualifications) must be provided, as well as biographies of any alternative member, if any.

Part 6 – Relevant Experience and Client References

In this section, a description of relevant work experience is to be provided, detailing the timeframe over which services were provided to specific clients. Contractors should provide a minimum of two (2) references and indicate the name of company, contact person, address, telephone number and role of the contact person in relation to the services provided. Information of relevance to the Scope of Work should be presented to allow EMSA to assess Contractor’s experience.

VII. LIMITATIONS

EMSA reserves the unqualified right to waive defects in any proposal, to choose or accept or reject any or all proposals submitted in response to this RFP for any reason whatsoever without explanation or justification, to select any firm as the party with whom it would be willing to enter into an agreement regardless of whether such party shall have submitted a proposal pursuant to this RFP at any time including the deletion, addition, modification or other changes hereto, and to make any decision in any manner as EMSA may choose in its sole discretion in the determination of its own interests and irrespective of the position of any interested party that in EMSA’s sole determination is unreasonable, unfair, arbitrary, discriminatory or capricious.

EMSA may consult references familiar with Contractor, regarding prior projects, management, financial resources, reputation or otherwise. Submission of a proposal in response to this RFP shall constitute permission for EMSA to make such inquiries and authorization to third parties to respond thereto.

EMSA makes no representations, warranties or guarantees that the information contained herein is accurate, complete or timely. The furnishing of such information by EMSA shall not create, nor be deemed to create any obligation or liability upon it for any reason whatsoever, and each organization by expressing its interest and submitting its proposal expressly agrees that it has not relied upon foregoing information and that it shall not hold EMSA liable or responsible therefore in any manner whatsoever.

VIII. public records requirements

EMSA shall hold all proposals and subsequent submissions in confidence, to the extent consistent with applicable law, until a final decision has been made or the selection process is terminated. Respondents are advised, however, that pursuant to Illinois law, all materials received by EMSA that fall within the definition of “public record”, shall be disclosed by EMSA upon request.
