Mt. Diablo Center, an Adult Day Health Care program of Choice in Aging located in Pleasant Hill, provides a supportive and safe environment, which maximizes the independence of adults

with disabilities, frail elders, and people with Alzheimer's disease or related dementias. In addition, education, advocacy and respite are provided for family caregivers.

Start Date: Immediately

Hours of Work: Full-Time Monday thru Friday 9:00 - 4:00 (Closed on all major holidays)

The Position: Provide health, maintenance and social day care services for the frail elderly, adults with disabilities, and adults with Alzheimer's disease and other related dementias. Travel to local residences and conduct the initial home assessment, coordinate the admission process, provide counseling to participants and families, conduct various meetings, maintain documentation, lead in-house support groups and act as a liaison to community resources, attend healthcare fairs, networking and outreach meetings.

Qualifications: * Applicant must possess a bachelor's degree in Social Work from an accredited school. * Two years of experience providing social services in one or more of the fields of ageing, health, or long-term care services. * Experience in a related field, demonstrated knowledge of case management, and knowledge of community resources for adults with disabilities and elderly preferred. * Ability to communicate in English in both verbal and written forms required. * TB Test, California Drivers License, own auto and insurance, clean DMV record, fingerprint clearance required.
