The plan of care for these services should reflect assessed family needs, as well as a clear delineation of services to be provided and the frequency of service delivery (up to 13 months following death of the client)Programs:All Midland Care Programs Person ResponsibleHOSPICE CLIENTSAt time of Admission to HospiceRN and/or Social Worker At initial visit, assess and identify potential bereavement risk factors (see Initial & Comprehensive Assessment procedure)Initiate Bereavement Plan of Care at admission in collaboration with the IDT and patient and family.Ongoing documentation will reflect intervention effectiveness, resolution of problems, identification of new problems, etc.Prior to Client DeathAll Team MembersOngoing assessment of grief and bereavement issues will be reflected in the documentation of all team members.Plan of Care will be updated as municate identified issues to Bereavement CounselorsTeam will work collaboratively in development of interventions for clients with complicated grief issues. This process will be evidenced in team meeting notes and client record documentationALL PROGRAMSAfter Client Death –Bereavement Counselor/ designeeLook at Census report daily for client deaths,Admit the deceased patient’s Primary Contact person to Bereavement. Bereavement services are available to persons of all ages, family members and significant others.Initiate Plan of Care for Condolence Card. Mail condolence card within 3 business days of patient’s death.Document correspondence in record.Make initial condolence call to family within 5 days of client’s death. Document on the Bereavement Risk Assessment to include any specific concerns. If caregiver is not available at time of call, document attempted condolence call on Bereavement assessment and proceed with bereavement admission if not already completed by BV Counselor. Bereavement Counselor/designeeWithin 4 weeks of the death, or sooner if indicated in the Bereavement Risk AssessmentMake the follow-up Support Call. Offer and encourage PCG to accept a home visit to introduce bereavement team. Set up visit (if desired.) If no visit is desired, educate about team, support and services available. Document and update POC.If desired, provide home visit per “model visit” including education about team, support and hospice survey that will arrive six weeks after the death. Document and update POC.If high risk/high need issues, complicated grief are identified, refer to Midland Care Social Worker for follow up. Individuals may receive up to 3 individual support sessions while in the bereavement program Individuals exhibiting complicated grief may receive services under Midland Care Grief and Loss Counseling and/or may be referred to community resources. Document in EMR.Provide information to the bereaved regarding availability of services which include individual, family and group support, camps, retreats, memorial services, special events and specialized groups.Administer the Grief Intensity Inventory and complete an initial assessment/interview to assess the level of emotional discomfort due to the death of their loved one for individuals making contact for participation in support groups or individual counseling. Assignment to group or individual counseling will be made based on the assessment finding and individual’s preference.If bereaved indicate no desire for additional interventions, document in the medical record. Discharge from Bereavement if requested. Support Specialist/designeeGenerate and assure that Bereavement Counselor/designee signs follow-up correspondence, i.e.: 2-month letter, 6-month letter, 9-month letter and 12 month/anniversary card. Mail letters.Document in the bereaved record.At the end of 13 months, discharge bereaved from bereavement program if no further services are required.All Team MembersIf bereaved expresses bereavement support needs beyond 13 months, assess needs and provide information regarding Midland Care’s Grief and Loss Services as well as community services available.
