Confirm that the site you are connected to:displays screens as shown in this document; and operates consistently with what is described in this document.Your username and passwordYour username and password play a critical part in securing the Data Gateway Public website.Your username and password must not be disclosed to anyone under any circumstances. Should the security and/or integrity of the username and/or password be compromised or if you suspect that the security and/or integrity of either might be compromised, the username and Password must be changed immediately. Your username and password are required to access your account as referred to in the “Authorized Users Only” statement on the Login Page.Access to the data gateway websiteLaunch your internet browser (Internet Firefox, Google Chrome, Edge etc.).Enter the URL address accessing the website this way, the application detects the language of your browser and displays the preferred language. If it fails to do so, it will display in English by default. It is always possible to change the language by using the link located at the top right of each page.NOTE: It is important to choose your language at the beginning of the session. Do not change the language while in the process of completing the death notification as sections of the form may become blank. Login This page will allow you to authenticate by entering your Username and password.Enter your username in the Username section. The username section is not case sensitive. Enter your password in the Password section. Note: this section is case sensitive. Select the Submit Authorization button to validate your username and password and to log into the system.NOTE: If you have forgotten your username and/or Password, you can select the Forgot your username and or Password link. If it is the first time you are connecting with your username, you will be required to do the following:agree to the Terms of Use;update your contact information via the My Account page (if required) ; andchange your password to a permanent one. Upon successful login and after taking action on the steps above (if required), you will be redirected to the Home Page in order to start completing the Enhanced Death Notification form. Completing the Death NotificationOnce you have logged into the Data Gateway, you will be directed to the Home Page. Obtain verbal consent from the executor and/or family to proceed with notifying Service Canada of the death of a CPP and/or OAS beneficiary. Note: If the executor and/or family decide not to have the funeral services provider assist in the completion of the death notification, please notify the EDN POC project team by emailing:EDSC.SASRPC.AvisdeDeces.PDC-POC.DeathNotification.CPPSIS.ESDC@servicecanada.gc.ca In the subject line of the email indicate: “Executor and/or family will notify Service Canada directly”. The body of the email can remain blank. Do not put any client identifying information in the email.Once verbal consent has been obtained, ensure that the dropdown under select an organization is the Funeral Services Provider that you are submitting the Enhanced Death Notification on behalf of. NOTE: If you work for a Funeral Services Provider with numerous locations, please select the Funeral Services Provider with your location. Select the button Submit Death Notification Form button.The Privacy Notification Statement will appear:Enter the name of the executor or family member providing consent to notify ESDC [Service Canada] and enter the date that the executor or family member provided the consent.Click the confirmation on the Privacy Notification Statement attesting that the executor and/or family have provided verbal consent to proceed with notifying Service Canada of the death of a CPP and/or OAS beneficiary to stop the payment of a CPP and/or OAS benefit in order to avoid an overpayment. You can activate the confirmation by clicking the bolded words or by clicking directly on the check box to the left of the bolded words.The death notification form template will appear:Complete the template in collaboration with the executor and/or family. Avoid using the space bar at the end of any section. If the space button is used, you will receive an error message when the template is submitted. All fields marked with an asterisk (*) are mandatory. NOTE: Validate that the Social Insurance Number (SIN) is in fact the SIN of the deceased. Ensure that the First and Last Name entered in the template matches the First and Last Name on the SIN card. Select the Submit button. Data Gateway will instantly validate the information against programmed edit rules. If there are errors or sections that are incomplete, you will be instructed to correct the fields:Fix any errors. When you have successfully submitted the Death Notification, you will instantly receive confirmation of receipt:Death Notification Portal – View HistoryYou can access the View History page from the Data Gateway Home page by clicking on the View History button.You will be directed to:You can search by session number or by date. Once you have completed either the session number or date fields click SearchYou can view the history as well by reviewing the entries. Deleting Only Enhanced Death Notifications (EDNs) that have not been transmitted can be deleted. The forms that have been transmitted to Service Canada cannot be deleted. If a form has not been transmitted due to errors or omissions, you will see: By clicking the trash icon you can delete the “non transmitted” form. NOTE: If a “non transmitted” form is not resubmitted within 7 calendar days, it will automatically be deleted. LogoutIt is important to logout of the Data Gateway once you are done. Select the Logout link from the left menu:You will receive notification that your session was terminated successfully.Features and other pagesTerms of useAgreeing to comply with the Terms of Use is a condition of using the Data Gateway. Please read them carefully. This page appears the first time you connect with your User ID. Should the Terms of Use be amended you will be required to agree to the amended version prior to using the Data Gateway. To continue, select I Agree and click on the Continue button. If you disagree with, cannot comply with, or are uncertain of the Terms of Use, select I disagree and contact us by emailing: EDSC.SASRPC.AvisdeDeces.PDC-POC.DeathNotification.CPPSIS.ESDC@servicecanada.gc.ca Account disabledIf you tried logging in using a non existing user ID more than 15 times, your access will be automatically locked and you will be redirected to this page. You will not be able to login again for another 30 minutes.If you tried logging in using a valid user ID, but an invalid password more than 3 times, your account will automatically be locked for a period of 30 minutes and will be redirected to this page. You can then wait for the 30 minutes to expire before trying to log in again, or refer to the Contact Us page of the website in order to have your account unlocked and/or your password reset.Forgot your Username and/or PasswordNOTE: You can only reset your password. You cannot retrieve your Username.In order to reset your password, you need the following:Your Username The email address registered to your accountIf you have forgotten your User ID and/or Email address registered to your account refer to the Contact Us page.If you have forgotten your Password, you can select the Forgot User ID and or Password link directly from the Login page.Selecting the link will bring you to: 56578526733500Select the second link Participant of other programs. Type in your Username in the Username field.Type in the email address registered for your account in the Email field.Select the Submit button.If the Username and email address provided are registered to your account, the system will notify you. You will receive an automated email from the system at the email address provided.7035807493000Note: Please allow a few minutes for the email message to come into your inbox. This automated email message will contain your new temporary password. If you did not receive the automated email message in your inbox, check your Spam or Junk email folder. If you still cannot find an email from the Data Gateway system, refer to the Contact Us page of the website for more information. Note: The email message will come from the sender nc-sys-data_gateway-passerelle_donnees-gd@hrsdc-rhdsc.gc.ca (DO NOT reply to the sender) and will look like the following:If the username and/or email address provided are invalid for your account, you will get the following message.
