|Home and Community Based Services provide a broad range of non-Medicaid-funded, |to the client at lunch/noon Monday thru Friday, |

|How does an individual obtain Home and Community Based Services? |How are services arranged? |

|The individual or their family, contact the Aging and Disability Resource |If funding is available for the requested services, the individual’s information |

What are the financial eligibility requirements?

There are no financial eligibility requirements for the home and community based funds, but

Home Delivered Meal participants are asked to make a voluntary client contribution to the provider towards

the cost of the meal. This is one way in which the programs are able to increase the number of persons they

can serve; and

Persons receiving Adult Day Care and In-Home Services such as personal care support, homemaker assistance,

and respite care are asked to pay for services on a sliding fee scale based on the household income and medical

expenses.

What amount of service can be obtained under the Home and Community Based funding?

Home Delivered Meals are offered once a day for the noon meal Monday thru Friday.

The amount of In-Home assistance offered is based on available funding and determined by the nurse at the

time of the individuals in home assessment. An average time in the home would be twice a week for one/

two hours each day.
